Join us and be part of our overnight nursing service! Help us shape and develop the service.We have an exciting opportunity for a Grade 3 Healthcare Support Worker or a candidate that would be willing to work towards a Grade 3, to join our team in Devizes, for 20 hours and 30 minutes a week.

Here at Wiltshire Health and Care we are very excited to announce the development and expansion of our community services in line with the NHS Long Term Plan and a number of strategic priorities for community health services to transform our out of hospital care. We are boosting the response capacity within the existing community teams to provide a dedicated resource across each of our localities within Wiltshire.
This growth within our services incorporates a consistent 2-hour crisis response element to our community services with appropriate skill mix of staff to support our current services across 12 hours (08:00 to 20:00) 7 days a week. We are also extending our services to provide nursing cover 24/7 in the community by developing an overnight element which will provide a much-needed service to our patients across Wiltshire. Overnight nursing shift times are 20:15 07:45.

Main duties of the job
- To implement treatment by following care plan.
- To keep the patient at the centre of his or her care, ensuring patient and carer participation in decision making.
- To undertake the daily care and support of delegated patients.
- Deliver treatment/care within a community setting as specified in a written care plan, under the guidance of a registered practitioner.
- Wherever possible to avoid hospital admission and reduce inpatient length of stay to ensure that patient receive the appropriate care in the most appropriate setting.
- Responsibility for the day-to-day care of patients including monitoring.
- Support the taking of medication under the authorisation of the qualified practitioner.
- To have a good knowledge of infection control, to reduce risk of health care acquired infections.
- To enable patients to reach their optimum level of independence.
- Support terminally ill patients and their carers/relatives.
- To communicate with individuals about routine and daily activities, this may include overcoming difficulties (e.g. hearing/language).
- To ensure the needs and wishes of the individual are documented.
- To work within a multi-professional team
- To undertake specific tasks as delegated.
- To assist registered and junior staff, referring to registered staff when necessary.
- To practice the principles and philosophy of individualised patient care in accordance with policy, procedures and regulatory bodys guidelines.
- To obtain consent for any treatment undertaken.
