Working closely with ourIntermediate Care Team, as a Community Rehab Assistant you will support and assist the senior team members toprovide care and rehab to people in their own homes .Our work is crucial to the community as we aim to prevent unnecessary hospital submission. In this role, you will care for patients in the way thatyou believe is best, delivering our high service standards, sharing bestpractice, and actively improving the way we work.
As part of our community nursing team we empower our staff to work autonomously and with flexibility by providing them with state of the artnew tabletsfor them to be able to check in from home, write their notes up on the go and allow them to not be trapped in an office base, so they can really enjoy working in their chosen community background!
We cover a 7-day service. We share out the shifts and cover the service from 8am - 8pm.
Main duties of the job
Within this role, you will:
- To assist in providing rehabilitation and care in patients own homes.
- Assist with home exercise programmes as prescribed by the therapy team.
- To maintain knowledge, skills and attitudes necessary to deliver a quality service that is responsive to the changing needs of patients.
- To provide clerical and administrative support to the team.
- To deliver care in accordance with organisation policies, procedures and guidelines, according to competencies, as delegated by OPS Lead/ Therapist.
- To report any changes in condition of the patient to the Operational Lead.
