Job summary

As a Senior Health Care Support Worker you will play a key role in delivering high-quality, person-centered care and services to support individuals and teams. Working within a collaborative environment, you will provide essential services tailored to meet the unique needs of those we serve. Based in Swindon, you will have the flexibility to work within a model that supports both on-site and hybrid working arrangements, depending on the nature of the role. HCRG Care Group is committed to fostering an agile and adaptable workforce to best meet the needs of our organization and service users.

Main duties of the job

To implement treatment by following care plan. To keep the patient at the centre of his or her care, ensuring patient and carer Working within a multi-professional team to provide delegated care. Delivering treatment and support as outlined in a care plan under the guidance of a registered practitioner. Keeping patients at the center of their care, ensuring active participation from both the patient and carers in decision-making. Supporting daily care needs for assigned patients in their home environments. Providing treatment and monitoring of patients as per care plans, following established guidelines and protocols.
