In this dynamic role, youll work closely with GPs, social care teams, and commissioners to ensure seamless communication and integrated care, empowering patients to manage their health and reduce unnecessary hospital admissions.
Youll collaborate with Multidisciplinary Teams to gather vital patient information, identify opportunities for preventative care, and develop and maintain care management plans that support independence and well-being. By championing the ESTHER Philosophy of Care, youll drive continuous quality improvement within the team and wider community.

This role involves travel to localities within the business unit meaninga driving license and access to a car for work purposes is required for this role.
Main duties of the job
As an MDT Coordinator your responsibilities will include:
- Attend and plan GP practice MDT meetings regularly, coordinating with the lead GP to ensure effective care planning and delivery.
- Support GP practice MDTs by facilitating care coordination, assessments, and information gathering for individual patients.
- Take and manage Health and Social Care referrals, conducting assessments via telephone or face-to-face as required.
- Develop and maintain strong relationships with GP practices, MDTs, and other agencies to ensure integrated and consistent care for service users.
- Act as the case manager for patients on the non-weight-bearing pathway.
- Work to reduce care home admissions by supporting carers and ensuring they have access to early services to prevent breakdown.
- Provide advice, guidance, and signposting to service users, helping them access public and voluntary sector resources.
- Ensure accurate and up-to-date patient records are maintained, using HCRG Care Group IT systems and health databases.
- Collaborate with community services to manage patients with long-term conditions and prevent unplanned hospital admissions.
- Participate in the development and evaluation of the Single Point of Access model and Integrated Neighbourhood Teams (INTs) to meet service and ICB requirements.
