Job summary

We are looking for aSenior Referral Clinician, whowill work within the Single Point of Access team at HCRG Care Group, working as part of a skill-mixed team, ensuring a high standard of service delivery.

As aSenior Referral Clinicianyou will be responsible for the digital screening of referrals to ensure effective prioritisation, safety, and accuracy. This involves making clinical decisions regarding the eligibility for access to services, applying a no wrong door approach to ensure inclusivity and seamless care pathways.

The role focuses on managing complex referrals, including those involving safeguarding concerns and intricate health needs, ensuring a robust and safe response to the needs of children and families.

The successful candidate will bring clinical expertise and substantial experience in childrens health and development.

As aSenior Referral Clinicianyou will clinically supervise and inspire the team to pursue excellence in care delivery, while providing clinical guidance and day-to-day leadership to uphold the quality and safety of services.

Base

The Single Point of Access function supports our Surrey-wide service, and you will be based at one of our Office Hubs across Surrey with the opportunity for hybrid working.

HCRG Care Group is a flexible-first employer, whilst you will have an office base, our teams are agile and work in flexible/hybrid arrangements.

Main duties of the job

Making clinical decisions on referrals about the eligibility for access to services.
Use Clinical expertise to triage referrals to the appropriate services Be able to liaise with services regarding referrals.
Have telephone conversations with parents, referrers and other partners in order to offer advice and guidance within your professional boundaries.
Support the team in service development and transformation.
Be aware of the continuous service changes and be able to apply these daily.
Discuss appropriate care pathways with referrers and families to understand their current needs, expectations of the service and to ensure utilisation of the most appropriate service.
Support clinical and day-to day leadership of the team including induction and development of new colleagues

Essential

A qualified Nurse or Allied Health Professional e.g. RCN, SCPHN, SLT, OT, Physio or equivalent, and clinical registration with a recognised professional body (e.g. NMC, HCPC)
Postgraduate/post-qualification experience working with Children and Young People for minimum of 3 years
Excellent communication skills, able to communicate with families, young people and other professionals verbally and in writing.
Exceptional clinical risk assessment and management skills. A sound understanding of current evidence and best practice in child health and care.

Desirable

1 year minimum experience of providing clinical supervision to others
Experience of working as a senior member of an MDT
A positive, motivated and passionatabout providing a high quality holistic approach to referral management.
