Job summary
We are looking for an enthusiastic and experienced Band 6 Staff Nurse or Therapist, with specialist knowledge and experience in discharge planning and pathways, to join our Community Transfer of Care Hub based at Gravesham Community Hospital.
Yourfocus will be to support palliative and EOL discharge for the Dartford, Gravesend and Swanley area. For this role you will be required to attend the local acute hospital to review and assess any palliative /EOL patients to ensure smooth discharge into the community, you will also be responsible to follow the identified patients after discharge and complete any Continuing Health Care checklists or Fast tracks. You must have anexcellent knowledge and understanding of palliative care.
Gravesham Community Hospital is located a short distance from St Georges Shopping Centre and is surrounded by well-known food establishments and local coffee shops making it an ideal location to be based from.
Whilst you will be based at Gravesham Community Hospital, you will be required to travel to local acute hospitals to provide in-reach support to meet the needs of the service and youll also be expected to visit patients in their homes and in care homes to undertake assessments in line with our "Home First" approach. Therefore, adriving license and access to a car for work purposes is essential for this role.
Main duties of the job
Main Responsibilities
As a Band 6 Team Sister / Therapist you will:
- Support and manage the Community Transfer of Care Hub Team on a day-to-day basis, providing clinical/management leadership, support, and guidance.
- As requested, support the Team Senior Sister and Clinical Service Manager with recruitment.
- Have an oversight of the bed management processes and reporting requirements
- Participate in the induction of new staff, students, bank, and agency staff.
- Involve patients and carers in the planning and delivery of community services/placements.
- Maximise best use of clinical resources and organise clinical cover where necessary.
- Monitor and evaluate patient flow, admission, and discharge arrangements, take responsibility for initiating improvements.
- Support members of the team with facilitating safe and timely discharge plans.
- Take responsibility for the most complex cases, including leading on the assessment of homeless patients ensuring that statutory responsibilities have been met.
- Reviewing complex patients that require 1-1 funding.
- Review and support with any safeguarding issues raised.
- Develop and support robust effective communication with all HCRG Care Group colleagues and other relevant agencies i.e., GPs, social care, nursing colleagues.
- Take clinical responsibility for any placements made by the Community Transfer of Care Hub and to deal with any issues arising from these placements.
