Job summary

TheAutism Outreach Serviceprovides comprehensive support to individuals, and families affected by Autism across Essex.

We prioritise personalised and family-centred care, ensuring that individuals receive the right support at the right time, in a coordinated and community-based manner.

Our team ofAutism Practitionersplays a vital role in delivering this vision, working closely with individuals, and their families to empower them and facilitate access to essential services.

As a compassionate and dedicatedAdult Autism Outreach Practitioner,the postholder will provide personalised support to individuals, and families affected by Autism.

TheAdult Autism Outreach Practitionerwill hold a caseload of individuals, working collaboratively with them and their families to develop personalised care plans, advocate for their needs, and ensure that they receive timely and coordinated support.

This role requires strong communication skills, creativity, and a commitment to empowering individuals to achieve greater independence and wellbeing.

Base

Essex-wide.

Area based but travel within the region may be required to fulfill the responsibilities of the role.

Main duties of the job

  • Work with a caseload of individuals to provide personalised support tailored to their needs and preferences.
  • Enable individuals to remain in the community whenever possible and prevent hospital admissions through proactive intervention and support
  • Engage with the individual and where required their family to develop a personalised, family-owned care plan that reflects their lived experience and preferences.
  • Ensure that care plans are flexible, responsive, and centered around the needs of the individual and their family.
  • Advocate on behalf of the individual and their family, facilitating open and transparent communication with other professionals and service providers.
  • Ensure that the voice of the individual, and where required their families are heard and respected in all discussions and decision-making processes.
  • Collaborate with other professionals and agencies to access relevant resources and support networks.
  • Identify and develop community assets and signpost families to additional support services as needed.

The Ideal Candidate
  • Relevant undergraduate degree, or evidence of working at degree level.
  • Strong communication and interpersonal skills, with the ability to engage effectively with individuals with diverse needs and backgrounds.
  • Ability to work collaboratively as part of a multidisciplinary team and build positive relationships with stakeholders.
  • An understanding of safeguarding issues and child protection procedures, identifying risk and when safeguarding protocols needs to be followed
  • Driving licence and access to vehicle for work purposes
