Job summary
We are looking for a permanent Band 5 Out of Hours Community Nurse to join our friendly, inclusive and forward thinking Out of Hours team, working across the West Lancashire area to provide care and support to patients in community settings.
The team is based from the Urgent Treatment Centre at Ormskirk Hospital. You will be working evenings on a 17.5 hour contract split across two weeks. Week 1 - 20 hours then Week 2 - 15 hours a week. Each shift is 5 hours long between the hours of 6pm and 11.30pm.
Main duties of the job
We are looking for a successful candidate who will be responsible for the following:
- To work as part of the 24-hour communitynursing service, under the direction of the Urgent Treatment CentreManager / Leader, providing high quality holistic care to patients.
- The OOH Community nurserole carries delegated responsibility for the assessment, planning, implementation and evaluation of evidence-based nursing to patients in their own homes.
- To assume delegated responsibility for the supervision of the team in the absence of the Urgent Treatment Centre Team Leader, when required.

You will have:
- A Degree/Diploma in Adult Nursing
- First level registered nurse
- Experience working as part of a multi-disciplinary team / multiagency working (Desirable)
- You need to have an understanding of undertaking complex assessments and the planning and evaluation of care package
- Excellent Communication, both written and verbal
- Evidence of effective interpersonal skills
- Clinical Risk assessment skills
- Ability to problem solve and make clinical decisions Understanding of NMC standards and implications to practice
- Ability to work independently in community setting
- High clinical standards
You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.
