Job summary

Were looking for Community Nurses tojoin our North Kent Community Nursing Service, Out of Hours Service, with a base at Livingstone Community Hospitalto help us continue to change things for the better. The community nursing team works across Dartford, Gravesend and outlying villages between 20.00-08.00.

Main duties of the job

As a Band 6 Out of Hours Nurse you will

  • Deliver nursing care to housebound adults within the community
  • Managing small team of nurses
  • Be responsible for contributing to the holistic assessment, planning delivery and evaluation of care to patients within their own homes.
  • Implement, monitor and maintain high standards of care delivery at all times.
  • Care for patientsand their families in the way thatyou believe is best
  • Deliver our high service standards, sharing bestpractice, and actively improving the way we work.

Job responsibilities

As an Out of Hours Nurse, youll be part of our valued team at Livingstone Community Hospital

You will be a NMC/1stlevel registered Nurse, you will have the ability to work both independently and also as part of a wider multidisciplinary team. It is expected that youshould have strongcommunication and organisation skills and a flexible approach which will ensureyou deliver positive experiences to patients at every stage of theirhealthcare journey.

You will have a full UK driving license with access to a car for work purposes.

You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.
