Deliver Care Where It Matters: As a Clinical Support Assistant, your role will be at the forefront of delivering exceptional care to patients in their own homes. You'll be part of our Virtual Ward clinical team focusing on preventing hospital admissions and facilitating early discharges. Our Virtual Ward Team is an NHS@Home Service step-up clinical team working in collaboration with the Royal United Hospital, Bath, who provide the step down aspect of the service.
You'll be part of a dynamic team that responds to patient needs with innovation and compassion. Youll work alongside forward-thinking colleagues who are dedicated to providing the highest quality care to our community and will be a vital part of our integrated community health and care services.
You will be based within the Care Coordination Centre in Peasedown St John but will work within the integrated community health & care services and peoples homes.
Main duties of the job
Key Responsibilities:
- Assist in the assessment, management, and treatment of patients in community settings, including therapy support.
- You'll work closely with patients, carers, and partners to ensure informed choices, perform phlebotomy, and maintain high standards of patient care.
- Ensure the efficient use of Virtual Ward equipment, keeping everything in top-notch condition.
- Maintain accurate and legible records of care, incorporating all relevant communication and liaison into your care plans.
