Job summary

An exciting opportunity has arisen to apply for the role of a Band 6/7 Stoma Care Clinical Nurse Specialist working in partnership with the East Lancashire Hospitals NHS Trust based in the community in the Blackburn area.

The successful applicant will work in the community providing specialist stoma care and support to patients discharged with a stoma from the Royal Blackburn Teaching hospital and surrounding area in support of the Acute based teams.

Main duties of the job

The post holder will develop and deliver a newly commissioned community-based stoma care service following a recognised care pathway delivering patient care in their own home.

The role will encompass:

  • providing specialist nursing care, information and advice to patients and their relatives
  • teaching and supporting other nursing and non-nursing staff and carers as required
  • liaising with GPs and community services

Job responsibilities

  • Provide specialist clinical advice on the safe provision of a stoma care nursing service
  • To be responsible for the safe and effective delivery of care within the designated area ensuring a high standard of patient care in liaison with stakeholders
  • evaluating and reviewing patterns of working, giving professional support where necessary
  • Ensure that care is provided in accordance with a structured pathway of care that is measurable and evidence based
  • To take a lead role in the training and development of staff and ensuring the needs of students are met
  • Ensure that the policies and practice procedures of Hollister and any other NHS Organisation are adhered to
  • Participate in discussion and the operation of research programmes affecting patients and the service
  • To be responsible for the delivery of service within the designated area within the agreed financial budget
  • Foster good communication links and co-operation between Community Nurses, Ward Managers and other disciplines and departments
  • Ensure timely and accurate entry of data into appropriate patient record keeping systems that meet the requirements of data protection and ensure that company liability is within current regulation
  • As part of the Nursing Team annual rota, provide occasional Weekend/Bank Holiday phone support services to the Secure Start Service. This will be a maximum of 3 weekend days (Sat/Sun) and no more than 1 bank holiday in any 12 month period)
  • Support the introduction of new products and provide opportunities for end users to review
  • Work with Company Nurse Manager to set annual clinical objectives and track regularly to ensure timely completion
