Job summary

Are you a dedicated, compassionate nurse looking for a fulfilling career in dementia care? At The Marbrook Centre we are looking for a passionate nurse to be our clinical lead for our residents living dementia.

Working as part of a small, friendly and competent team, working on our dementia service you will have clinical oversight of our residents individual needs with the support of a qualified/ non-qualified nurse management team.

Main duties of the job

With strong communication skills, you will be hands on and be involved in every aspect of our residents care - ranging from personal care to managing and administering medication.

You will ensure all residents are provided with the highest possible standards of care and support, including end of life care and will build trusting and transparent relationships with their families, identifying care needs and preferences and ensure these are accurately reflected in care plans as appropriate. You will also liaise with other health professionals regarding health care needs and make requests for GP/Professional visits where necessary. Setting example through your clinical knowledge and professionalism you will role model best practise and inspire and mentor junior staff.

Previous experience of dementia and end of life care would be an advantage. You will also be part of the on-call rota.

Job responsibilities

Purpose of role:

1. To ensure clinical care is delivered and available to residents, and that care workers are safely and appropriately overseen and supported during shift for clinical matters. To assess, plan and implement care, providing nursing advice and carry out nursing procedures as required.

Specific Responsibilities:

To be directly responsible to the Service Manager

To work closely with the GP and other members of the wider multiple disciplinary team

To provide care to a high standard in the following:

1. Experience or knowledge of neurological/dementia and physical disabilities.

2. Undertake clinical assessments of residents, including wound care and pressure area, continence assessments

3. Catheterisation

4. Diabetes management

5. Clinical Observations

6. Venepuncture

7. End of life care

8. Care planning

9. Bowel management

10. Assess and manage on going risks as identified during all resident contact and ensure that this is done in collaboration with our residents and their loved ones. To regularly review risk factors and make changes to the management of them as necessary

11. To undertake pre-admission assessments as requested by or in conjunction with the service manager.

To deliver training and support to care staff in delegated tasks as per general/service manager guidance

To participate in the on-call rota shared with other Nurses/Support Team Managers

To maintain up to date evidence based practice
