Job responsibilities

The Post Holder will be responsible for monitoring the physical health of patients within Cygnet Hospital Bury, working closely with the Lead physical ah Health nurse.

The role will involve co-ordinating the GP clinic on a weekly basis, taking responsibility for referring patients if other health professional involvement is indicated, ensuring that documentation is completed i.e. GP notes, physical health monitoring forms and Physical Health Care Plans. The role will involve liaising with the teams on each clinical area, responding to the educational needs of patients and becoming involved in the training of staff as required.

Clinical:

The post holder will:

  • Triage referrals made by the wards for GP appointments and prioritise according to needs.
  • Co-ordinate the GP clinic on a weekly basis.
  • Co-ordinate the Blood Clinic on a weekly basis inc. Clozaril and Lithium monitoring.
  • Liaise with relevant health professionals e.g. dietician, optician, chiropodist, continence nurse, dentist, Tissue viability nurse, Continence team, Community diabetes specialist team, etc.,
  • Closely monitor patient health issues such as obesity, hypertension, diabetes, epilepsy, asthma, COPD, minor injuries and illnesses.
  • Six monthly and annual health review of patients
  • Carry out ECGs.
  • Carry out Metabolic Syndrome monitoring.
  • Co-ordinate with GP surgery for following routine reviews: cervical smear screening, breast cancer screening, annual chronic health review, etc.,
  • Develop education groups / provide information to patients inc. healthy living groups, smoking cessation sessions, well woman / man clinics, drug and alcohol awareness with a view to promoting health, recovery and independence.
  • Liaise with pharmacist and contribute to medicines management committee.
  • Run vaccination clinics for patients (and staff).
  • Liaise with Infection Prevention lead.
  • Attend CPAs / Ward Reviews, SIRs where necessary.
  • Contribute to CPA reports and Liaison reports.
  • Check emergency equipment and contribute to CPR drills / training.
  • Support mental health nursing staff in managing minor injuries and wound care management.
  • Assist with Gym assessments.
  • Model good practice and contribute to the provision of care by providing specialist therapeutic input where appropriate.
  • Maintain accurate, detailed reports and records.
  • Consult and coordinate with Multidisciplinary team members to assess, plan, implement and evaluate patient care.

Managerial / Leadership:

  • Contribute to the clinical Governance Process.
  • Provide reports for Quality Reports and to ensure CQUIN targets are achieved.
  • Develop audits which ensure essential standards are adhered to and where necessary develop action plans to maintain good standards.
  • Communicate in timely way with the latest relevant information.
  • Contribute to relevant policies and procedures.
  • Advice staff as required to continue to maintain quality physical health monitoring in absence of physical health team during out of hours.
  • Assist staff to devise quality physical health care plans based on best practice developed through research and evidence.
  • Contribute to Clinical Governance and Medicines Management Committee.

Training:

  • Contribute to Induction of new starters during induction programme.
  • Develop relevant clinical skills training for staff in collaboration with the training department and external training agencies if appropriate.
  • Keep abreast of latest research and advice on evidence based practise ensuring that national initiatives are considered.
  • Be innovative around encouraging patients to become involved in training packages for staff / patients.
  • Take own responsibility for completing mandatory training and CPDs as required for the role.
