Job summary

PICS are looking for an enthusiastic, confident, flexible and highly organised person to work within Byron Primary Care Network (PCN) Team to help increase the number of health checks conducted within the PCN.This role is working 7.5 hours per week flexibly across weekdays and weekends on a fixed term basis for 4 months.

You will be based within Byron Primary Care Network (PCN) covering:

  • Whyburn Medical Practice, NG15 7JE
  • Oakenhall Medical Practice, NG15 7UA
  • The Om Surgery, NG15 7JP
  • Torkard Hill Medical Centre, NG15 6DY

Byron PCN has a patient population in excess of 35,000.

Main duties of the job

KEY RESPONSIBILITIES

  • Requirements will be to attend local areas ie supermarkets and local shops and offer the opportunity for a one stop screening processes.
  • This will include completing a simple questionnaire based on alcohol consumption and other lifestyle factors as well as performing a BP measurement and a finger prick blood test using the PocDoc product.
  • Patients who are identified will also be offered the opportunity to have the kit mailed out to them and perform the required checks at home.
  • Following this it will be required to input all the data onto the clinical record of the patient.
  • There will also be the requirement to present the work at both a local and a national level.
  • Utilise Population Health Intelligence tools where needed to support other patients in the PCN.

KEY REQUIREMENTS

  • Good standard of education GCSE or equivalent in English and Mathematics
  • Have ICT skills and knowledge of general software applications relevant to the post, including Word, Excel and management information systems.
  • Proven administrative experience in a complex and demanding environment.

Job responsibilities

Role objectives:

The post holder, working closely with the PCN and practice team to:

  • Requirements will be to attend local areas ie supermarkets and local shops and offer the opportunity for a one stop screening processes.
  • This will include completing a simple questionnaire based on alcohol consumption and other lifestyle factors as well as performing a BP measurement and a finger prick blood test using the PocDoc product.

Additional Responsibilities

  • Be responsible for using e-healthscope to identify eligible patients for a health check offer.
  • Contacting them to offer health checks.
  • Where time permits helping with inputting work related to the PCNs healthy hearts events.
  • To be responsible for recording, reporting and producing evaluation reports which will include evaluation detailing effectiveness outcomes of new roles.
  • To be customer (patient, carer, GP) focused when representing the service
  • To work collaboratively with other teams and services to maintain an effective and efficient service
  • To offer appropriate support and guidance to patients and their families/carers
  • To plan / organise work using own initiative, whilst being able to work as a valuable member of a team
  • To have excellent IT skills, to include Microsoft Office, Outlook and Excel
  • To undertake general office duties to support the role
  • To work effectively as part of a team and to be flexible regarding working hours to meet the needs of the service
  • To ensure all electronic records are updated and complete within the agreed timescales
  • To use a range of verbal and non-verbal communication tools to communicate effectively with patients, carers and families and colleague
  • Provide coordination of and participate in relevant internal and external working groups and provide project advice, expertise and support where requested
  • Support the PCN team by inputting into the overall strategy development and programming of work streams by applying knowledge and understanding of programme and project management
  • Work with key personnel in the PCN to develop & support collective general practice projects including areas of federated working
  • Work towards completing the appropriate mandatory Care Coordination training to deliver and support the Comprehensive Model for Personalised Care
  • Provide coordination of and participate in relevant internal and external working groups and provide project advice, expertise and support where requested
  • To participate in the review and appraisal process
  • To carry out any other reasonable duties as requested by a manager to ensure quality of service
  • To participate in any relevant training/courses/conferences
  • Complete mandatory training
  • Use clinical systems for record keeping, audit and evaluation
  • Develop and implement data collection systems that will provide accurate and timely data.
