Job summary
We have an exciting opportunity for a PCN Care Co-ordinators to join our Primary Care team. Arrow Health Primary Care Network (PCN) are looking for enthusiastic and highly organised individuals to work within the team providing co-ordination, administrative and support to The Ivy Medical Group. The role will be based within the GP Surgery.
This role will help to deliver enhanced care and support to the local population.

- Apple Tree Medical Practice, 4 Wheatsheaf Court, Burton Joyce, Nottingham, NG14 5EA
- Lambley Lane Surgery, 6 Lambley Lane, Burton Joyce, NG14 5BG
Main duties of the job
MAIN DUTIES OF THE JOB
- Making and managing appointments and making sure that patients have understandable written or verbal information to help them make choices about their care which will address any health in equality.
- Run reports from clinical systems such as SystmOne
- Work with teams within the GP practice and those who support GP practices
- Good standard of education GCSE or equivalent in English and Mathematics
- Have ICT skills and knowledge of general software applications relevant to the post, including Word, Excel and management information systems.
- Proven administrative experience in a complex and demanding environment.
