Job summary
Given the challenges for General Practice we are looking to try and centralise a number of functions within the practices but also to standardise processes and policies to improve efficiency and reduce risk. This includes the management of medications, prescriptions and reviews within the practices.
The post holder is a clinical pharmacist, who acts within their professional boundaries, supporting and working with the Medical and Deputy Medical Director as well as the practices to support and lead on the co-creation, design and implementation of the new medicines management strategy.
Main duties of the job
KEY RESPONSIBILITIES
- Contribute pharmaceutical advice for the development and implementation of new services that have medicinal components (e.g., advice on treatment pathways and patient information leaflets).
- Analyse, interpret, and present medicines data to highlight issues and risks to support decision making.
- Produce and implement a practice repeat prescribing policy. Manage the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates and flagging up those needing a review. Ensure patients have appropriate monitoring tests in place when required.
KEY REQUIREMENTS
- Mandatory registration with General Pharmaceutical Council (GPhC)
- Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
- Masters degree in Pharmacy (MPharm)
