Job summary
We are seeking
a qualified Clinical Pharmacist (CP) to work as part of a multi-disciplinary team in
general practice in a patient facing role to support all aspects of
primary care prescribing and medicines use. You would be joining an established pharmacy team working closely with GPs, nurses and other practice staff.
The
team is highly regarded, and we are looking to further increase our team to
help patients get the best out of their medicines and reduce the demands on
general practitioners.
We

to make the best use of your existing skills and experience
It
is a condition of this role that you are enrolled on the CPPE
Primary Care Pharmacy Education Pathway or has completed one of the earlier
iterations of this training pathway. This is an 18-month period of study with a
modular format. Study leave is given to participate but it is expected that the
you will complete some of the study materials and Portfolio entries
within their own time. On completion of the CPPE Pathway you will have the opportunity
to complete the training necessary to become an Independent Prescriber.

Main duties of the job
KEY
RESPONSIBILITIES
- Undertake
medication reviews and structured medication reviews with patients, both face
to face and remotely
- Take
part in the multi-disciplinary team meetings to support the care of people
living in care homes.
- Support patients with single or multiple medical
problems where medicine optimisation is required
- To integrate the practice with the community and
hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure
better access to healthcare, and help manage workload

PURPOSE
The pharmacist will work in general practice in a patient facing role to
support all aspects of primary care prescribing and medicines use. They will
work closely with GPs, nurses, and practice staff as part of the PCN Pharmacy
team of clinical pharmacists and technicians.
KEY
RESPONSIBILITIES
- Undertake
medication reviews and structured medication reviews with patients, both face
to face and by telephone.
- Undertake
medicines reconciliation on receipt of discharge summaries or clinic letters,
or other transfers of care. Make or check changes made to repeat prescriptions made
by others in accordance with local protocols.
- Take
part in the multi-disciplinary team meetings to support the care of people
living in care homes.
- You
will use your consultation skills to work in partnership with patients to
ensure they use their medicines effectively.
- Oversee
the monitoring of patients on higher risk medicines, including Shared Care
medicines e.g. methotrexate, lithium, amiodarone
- Contribute
to practice processes that ensure safe medicine use and management of long-term
conditions e.g. recall systems and support achievement of QoF domains and
disease registers.
- Respond
to queries from GPs, nurses, practice staff, patients, care home staff and
pharmacies regarding medicines, within your competency.
- You
will have the opportunity to run patient clinics to review long term conditions,
within your competency.
- Undertake
audits, quality improvement projects, develop services and initiatives to
improve the safety, quality and cost effectiveness of prescribing
- Work
with the practice and PCN multi-disciplinary teams to ensure efficient and safe
medicine optimisation processes are in place and being followed.
- Work
to reduce risk of medication incidents through review of practice processes,
ensuring appropriate reporting.

expertise to address both the public health and social care needs of patients,
including lifestyle advice, service information and help in tackling health inequalities

and participate in shared learning across the practice, PCN, PICs and wider

to unpredictable work patterns and urgent requests within a reasonable time
frame.

up to date with Nottinghamshire APC medicines formulary and local and national

update and maintain accurate patient medication records on the practice
clinical computer system, including advice given and action taken
