Job summary
We have an exciting opportunity to join our PCN team working closely with our Mental Health Occupational Therapist and Mental Health Practitioners to play a crucial role in optimising medication in patients with mental health conditions. Your expertise will contribute to the overall well-being of our community by ensuring safe, effective, and patient-centred pharmaceutical care. The role will involve conducting in depth medication reviews, collaborating with the MDT to provide personalised treatment plans, and monitoring medication adherence and efficacy, adjusting therapies as necessary.
A candidate with previous experience in primary care or mental health is highly desirable, but not essential.
As a Clinical Pharmacist, you will act within your professional boundaries, supporting and working alongside a team of Pharmacists in General Practice. In this role, you will be supported by a Senior Clinical Pharmacist, Clinical Lead and GP who will develop, manage, and mentor them.
Main duties of the job
KEY RESPONSIBILITIES
- Undertake clinical medication reviews with patients and produce recommendations for Senior Clinical Pharmacist, Nurses and/or GP on prescribing and monitoring.
- Managing caseload for patients with common/minor/self-limiting ailments while working within a scope of practice and limits of competence. Signposting to community pharmacy and referring to GPs or other healthcare professionals where appropriate
- Ensure that patients are referred to the appropriate Healthcare Professional for the appropriate level of care within an appropriate period of time e.g. pathology results, common/minor ailments, acute conditions, long term condition reviews etc.
KEY REQUIREMENTS
- Mandatory registration with General Pharmaceutical Council
- Masters degree in Pharmacy (MPharm)
- Minimum of 2 years postqualification experience.
