Job summary
We are seeking a qualified Clinical Pharmacist to work as part of a multi-disciplinary team in general practice in a patient facing role to support all aspects of primary care prescribing and medicines use. You would be joining an established Pharmacy team of Clinical Pharmacists, technicians and a Senior Clinical Pharmacist working closely with GPs, nurses, and practice staff as part of the PCN Pharmacy team.
The team is highly regarded, and we are looking to further increase our team to help patients get the best out of their medicines and reduce the demands on general practitioners.
It is a condition of this role that the post-holder is enrolled on the CPPE Primary Care Pharmacy Education Pathway or has completed one of the earlier iterations of this training pathway. This is an 18-month period of study with a modular format. Study leave is given to participate but it is expected that the post-holder will complete some of the study materials and Portfolio entries within their own time. On completion of the CPPE Pathway you will have the opportunity to complete the training necessary to become an Independent Prescriber.
You will be based within Mansfield North Primary Care Network (PCN) covering our 7 practices as follows;

- Pleasley Surgery NG19 7PE
- Riverbank Medical Services NG20 0BP
- Meden Medical Services NG20 0BP
- Sandy Lane Surgery NG18 2LT
- St Peters Medical Practice NG18 1EE
Main duties of the job
KEY RESPONSIBILITIES
- Undertake medication reviews and structured medication reviews with patients, both face to face and remotely
- Take part in the multi-disciplinary team meetings to support the care of people living in care homes.
- Support patients with single or multiple medical problems where medicine optimisation is required
- To integrate the practice with the community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare, and help manage workload
KEY REQUIREMENTS
- Masters in Pharmacy (MPHarm)
- Registered Pharmacist with General Pharmaceutical Council
- Experience of working as a registered Pharmacist
