The Cardiovascular Primary Prevention Pharmacy led service is a radical and unique PCN wide service that has been nominated for national awards. It aims to improve hypertension case finding and management, alongside reduction of cardiovascular risk for the PCN population

Nottingham West PCN have an exciting opportunity for a Pharmacy Technician to join our dynamic team. We are looking for someone who is enthusiastic about providing high quality care.

Nottingham West Primary Care Network is developing a pharmacy lead cardiology service, focusing on primary prevention initially focusing on undiagnosed Hypertension and Hyperlipidaemia, expanding to prediabetes and Chronic Kidney Disease. The cardiology pharmacy technician role will be integral to this service and key to the development of the cardiology pharmacy team.

At PICS, patient safety is at the core of everything we do and we recognise the importance of the support provided by Pharmacy Technicians in improving best practice to maximise safe and cost effective prescribing. Our Senior Pharmacy staff will support you to fulfil the requirements of the role and you will have access to the CPPE pathway in order to develop your skills. We are committed to further the development of Pharmacy Technicians within Primary Care.

You will be part of a wider team of Pharmacy Technicians and supported by our Clinical Pharmacist Clinical Lead who is on the PICS leadership team.

Main duties of the job

KEY RESPONSIBILITIES

  • Be a key player in the PCN Pharmacist Lead Cardiology team
  • Support the Clinical Pharmacist in identification of clinically appropriate patients to be under the cardiology service.
  • Carry out medicines optimisation reviews and utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively.
  • Undertake telephone consultations and patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients
  • Support patients with lifestyle advice, smoking cessation and obesity management.
  • Support the clinical pharmacist around identification and ordering of cardiology tests, including blood tests
  • Support the clinical pharmacist to ensure all patients are followed up in a clinically appropriate manner.
  • Implement and be responsible for efficient identification of suitable patients and communication to patients around the need for referral into the cardiology service.

KEY REQUIREMENTS

  • General Pharmaceutical Council professional registration
  • Good clinical knowledge (including terminology) and knowledge and understanding of pharmacy law, ethics and current legislation
  • Evidence of continued professional development

Nottingham West PCN is a group of GP practices (12) that are committed to work together to add additional services and roles to support the workload in GP practices. We cover 108,865 patients within our locality. Our network works closely with all practices and other key members of the network including pharmacies, Citizens Advice, Voluntary Sector and Local District and Borough Councils. Our aim is to continually improve services to patients utilising all available resources.

Our PCN work in partnership with PICS to provide a host organisation for employment contracts, HR and corporate support. The day-to-day work is situated in the GP practices and the everyday line management is undertaken by staff within the Primary Care Network.

JOB PURPOSE

Pharmacy Technicians play an important role, complementing Clinical Pharmacists, Community Pharmacists, and other members of the PCN multi-disciplinary team. Pharmacy Technicians are different to Clinical Pharmacists as they are not able to prescribe or make clinical decisions, instead working under supervision to ensure effective and efficient use of medicines.

The purpose of the role is to work within a multi-disciplinary team that focuses on Cardiology Primary and Secondary prevention. The post holder will work with colleagues to support the identification of patients with undiagnosed Hypertension, Atrial Fibrillation and Hyperlipidaemia.

The post holder will support the team to maximise safe, cost effective best practice in prescribing and supporting patients with lifestyle changes to improve the quality of patient care. The post holder will also help patients to get the best from their medicines by switching medications under agreed and approved protocols.
