Job summary
Nottingham West Primary Care Network (PCN) are recruiting 6 Clinical Pharmacists into newly created roles. You will be an experienced Pharmacist who will act within their professional boundaries supporting and working alongside a team of Pharmacists in General Practice and supported by a Neighbourhood Lead.
Working as part of a multi-disciplinary team in a patient facing role, you will be responsible for chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
You will provide primary support to general practice staff with regards to prescription and medication queries by supporting the repeat prescriptions system, dealing with acute prescription requests, and medicine reconciliation and support the IIF targets.

Main duties of the job
Key Responsibilities
- Work as a multi-disciplinary team in a patient facing role
- Provide primary support to general practice staff with regards to prescription and medication queries
- Provide clinical leadership on medicines optimisation and quality improvement
Key Requirements
- Master's degree in Pharmacy (MPharm)
- Mandatory professional registration with General Pharmaceutical Council
- Minimum of 2 years post-qualification experience
- Knowledge and understanding of the nature of GP Practices

Training and development
