Job summary

Nottingham West PCN have an exciting opportunity for anyone who is keen to be at the front line of healthcare development and engagement. We are looking to appoint two digital inclusion officers to join our dynamic team. These are brand new roles to the NHS, designed to develop engagement and support digital inclusion for every member of our community. We are seeking to appoint two enthusiastic, tech-savvy, individuals who care about everyone having the same equitable quality access to primary care services across Nottingham West PCN. If successful, you will support both citizens and primary care staff to understand, access and utilise different digital platforms to get the very best from their local health services as well as collaborating with wider PCN partners to introduce citizens to other services designed to enhance their whole wellbeing. You will work across 12 GP practices, and with Broxtowe Borough Council, Voluntary sector partners and patient engagement groups to help to reduce health inequalities and promote digital inclusion for all.

You will be based within Nottingham West PCN covering:

  • Beeston neighbourhood
  • Eastwood neighbourhood
  • Stapleford neighbourhood

Main duties of the job

KEY RESPONSIBILITIES

  • To identify and support patients of the PCN to access digital platforms
  • To increase awareness of digital patient platforms
  • To promote digital health tools and promote those available to patient groups

KEY REQUIREMENTS

  • GCSE English or equivalent level
  • GCSE Mathematics or equivalent level
  • Evidence of continuous professional development

Job responsibilities

JOB PURPOSE

Digital Inclusion Officers play an important role supporting citizens across Nottingham West PCN to digitally access primary care services and to support citizens to build confidence accessing digital platforms and increasing awareness of digital health solutions. As a member of the PCN multi-disciplinary team, you will focus on supporting those members of our community who are currently digitally excluded, to access and benefit from, the many digital solutions available to them across the Primary Care Network.

The post holder will work with colleagues to support the identification of citizens who are currently unable to access digital solutions, in addition, the post holder will encourage citizens to develop a better understanding of the principles of digital health care and further opportunities available to support their wider health and wellbeing.

This will involve working with Nottingham West citizens and practice staff to make registering with a GP surgery easier and to help citizens understand how to make the best use of online services. The post holder will play a key role in helping to reducing health inequalities across Nottingham West PCN by increasing digital inclusion for all citizens. This in turn will support practice staff by reducing the burden of administrative tasks as well as empowering citizens to feel more in control of their healthcare through digital access.

The post holder will work in practices across Nottingham West, as well as liaising with other local providers, such as the Broxtowe Borough Council and voluntary sector providers to set up outreach clinics in libraries/health centres and community facilities. The role requires someone to be engaging, personable, work as a team and independently, to explore current digital options and support new ways of working with patients and staff to improve access to health services.

KEY RESPONSIBILITIES

  • Contribute to the positive promotion of digital health tools and readily promote and support access of digital health tools available, including the NHS App and Patients Know Best to citizens and colleagues in the primary care network (PCN).
  • To help patients manage their health, care, and wellbeing through improved access to services.
  • Work with practices and practice staff to identify, reduce and remove the barriers to digital inclusion for people that experience the greatest health inequalities.
  • Support and develop digital champions across practices and being a point of contact and developing a central point of contact and knowledge.
  • Support the facilitation of workshops and task and finish groups to reach those in greatest need of support. Utilise communications and presentation skills to engage people and present difficult issues clearly to a range of audiences.
  • Support the development of stakeholder communication such as newsletters and briefings. As well as provide case to promote the services and a platform for further service improvement.
  • Responsible for managing and co-ordinating, with support from the PCN, the delivery of the digital inclusion project across the PCN.
  • To support with project schemes and initiatives at the planning, development, and implementation stages. There is a key focus on reducing health inequalities, those who find digital access difficult and supporting those seeking asylum within Nottingham West / Broxtowe Borough.
  • Responsible for monitoring the progress of the digital inclusion project objectives and ensure that reporting of actions takes place. Then using this information to advise on, initiate and develop services as appropriate with system colleagues across health, social care, voluntary services, and patient groups.
  • Evaluate the impact of the project’s schemes and track and monitor post implementation benefits.
  • Build and maintain networks working effectively with local colleagues to deliver the objectives of the role.
  • Complies with the policies and procedures of the organisation.
  • Recognise the roles of other colleagues within the organisation and their role to patient care
  • Demonstrate use of appropriate communication to gain the co-operation of relevant stakeholders (including patients, senior and peer colleagues, and other professionals, other NHS/private organisations e.g., CCGs)
