JOB
PURPOSE
As an
experienced Occupational Therapist with a mental health background, you will
provide professional leadership, support and advice within the
multi-disciplinary team across a range of Primary Care practices. The post
holder will:
- Work across
organisational boundaries, developing new way of working to promote
relationships to increase the quality of patient care and service within
primary care.
- Provide a
high level of occupational therapy/professional care and treatment to patients
through high quality occupational therapy care, assessment and therapeutic
intervention to adults with complex and substantial difficulties as part of a
multidisciplinary team approach.
- Work
autonomously to undertake advanced practice procedures and promote recovery and
wellbeing and maximising independence.
- Facilitate
mainstream primary care services for adults with mental health difficulties to
advise and support agencies with regard to meeting their duty of care.
- Be
responsible for disseminating information about good practice and lead on the
implementation of good practice that has been developed within the Primary Care
Network.
- Provide
compassionate care that is based on empathy, kindness, respect and dignity.
- Participate
in the planning, development and evaluation of clinical services leading on
defined projects as agreed with the Clinical Director and in collaboration with
the multi-disciplinary team.

Professional
Leadership and Development
- To be responsible
for maintaining own competency to practice through Continuing Professional
Development activities and to participate in the review and appraisal process.
- To adhere to the individual
Professional code of conduct and practice at all times.
- To reflect on own practice
individually and/or through regular clinical supervision.
- Provide clinical education, training
and evaluation of clinical competence of team and multidisciplinary team staff,
providing support and guidance.
- To maintain, develop and disseminate
specialist knowledge of evidence-based practice.
Communication
and Relationships
- Demonstrate advanced
communication skills in providing advice, empowerment, facilitation, influence,
motivation and counselling.
- Manage complex
communication issues that may arise between differing health and social care
professionals including de-escalation skills when working in highly
antagonistic, hostile and emotive clinical areas.
- Demonstrate an
understanding of the need to involve patients and carers in decision making,
offering them informed choices, respecting their views.
- Demonstrate
the ability to recognise barriers to understanding that can be encountered with
those without English as their first language, patients with disabilities and
those with mental health difficulties such as anxiety, clinical depression or
personality disorders.
- Seek
out different styles and methods of communicating to assist longer term needs
and aims.
Knowledge,
Skills, Training and Experience
- Occupational Therapy qualification HCPC registered
occupational therapist.
- Post graduate degree or equivalent qualification or
experience.
- A teaching qualification (e.g., ENB 998/P126/127) or
a proven track record in occupational therapy education is desirable but not
essential.
- Highly developed specialist knowledge of meeting the
health needs of Adults with Mental Health underpinned by theory and experience.
- In depth knowledge of National and HCPC policies,
procedures, code of conduct and standards of care in relation to service
provision for adults with mental health.
Analytical
and judgment skills
- Assess, interpret and analyse range
highly complex clinical information and environments, delivering a clear
rational for care delivery, clinical outcomes and clinical management
strategies.
- Demonstrate a high level of
reflective practice and competence in clinical reasoning and decision making
based on evidence-based nursing practice.
- Demonstrate excellent analytical and
judgmental skills in relation to Initial and on-going assessments and
subsequent treatment packages and assist colleagues in problem solving and
providing quality care and make recommendations as to the best course of action.
- Initiate and promote Serious Untoward
Incident (SUI) reporting and contribute to SUI Investigations as Lead Clinician
or Investigator.
- Contribute, Support and Lead on
Complaints Resolutions where the Complaint is in relation to the Quality of
clinical interventions and Treatment received. Take on role as Investigating
Officer for formal complaints.
Planning
and organisation skills
- Responsible for planning and
prioritising their own workload.
- Work independently as an Advanced
Practitioner to manage own complex caseload.
- Plan and organise specialist
occupational therapy provision and adjust as required.
- Promote, develop, and implement
clinical standards and good practices both in care delivery and appropriate
record keeping.
- Represent in the absence of the
Clinical Team Lead as appropriate.
Responsibilities
for patient/client care
- Following the completion of a
comprehensive core assessment, develop and provide an individualised, evidence-based
programme of care based on formulation, including to those with complex
needs/challenging behaviours.
- At all times work in collaboration
with patients and carers ensuring that the patient is treated with compassion,
respect and dignity.
- Manage distress and conflict in the day-to-day
work with patients who, as an expression of their condition/ diagnosis, may at
times be difficult to engage and/or demonstrate significant risk behaviours.
- Provide effective care and
therapeutic interventions to patients presenting with a range of difficulties.
To engage in active relapse prevention planning with patients and carers,
contributing to their recovery and/ or wellbeing and discharge planning.
- Provide specialist advice and
guidance to patients/carers and practitioners of other professional
disciplines, less experienced staff, voluntary organisations and the public.
Policy
and Service Development
- Adhere to policies, procedures, protocols and
standards and be instrumental in embedding these in service provision.
- Adhere to professional code of conduct and standards,
and requirements of other legislation.
- Understand, contribute and advise on statutory
issues which impact on patients and carers. Perform statutory duties and seek
advice where necessary.
- Positively promote adherence to Lone
Working Policy and Protocols.
- Ensure Implementation of National,
Trust and Local Service Legislation, Policies, Procedures and Guidelines for
Health and Social Care and be involved in their development where appropriate.
Report any deficiencies within the clinical environment or equipment.
Human
Resources
- Take responsibility for keeping own professional
knowledge and skills up to date through mandatory training, reflective
practice, shared learning, etc.
- Be responsible for being actively involved in CPD
and professional development plans via supervision and appraisal process.
- Completion of the HEE First Contact Practitioner
Roadmap with the support of the Occupational Therapy Clinical Lead.
Research
and Development
- Initiate and Lead on regular Clinical
Audit and participate in Research as required.
- Keep up to date with relevant
research in the field in order to evaluate current practice and suggest service
improvement.
- Share any knowledge gained via
conference, peer review journals, specific training sessions, workshops,
presentations etc. in order to develop and underpin good practice.
- To reflect on own practice through
clinical supervision and mentorship and to develop skills as a clinical
supervisor/mentor to others.
- To contribute to the development of
local evidence-based standards, policies and guidelines related to the
speciality.
Clinical Governance
- To maintain accurate and comprehensive
documentation in line with legal, professional and organisational requirements.
Ensure all information with regard to each patient related contact is recorded
within SystmOne records
- Identify
need for, and undertake, research, clinical audit, benchmarking and equipment
trials, in order to improve effectiveness of patient care.
- Disseminate
research and audit findings through presentation to multi- professional groups
and publication.
- Take
responsibility for the development and implementation of policies, procedures
and guidelines relevant to own area of work, which may have an impact across
the organisation.
- Promote
patient and public involvement activities in the specialist area, leading to
service improvement.
Professional
Development and Personal Performance
- Identify
learning needs, plan, implement and evaluate programmes of education to meet
identified need.
- Establish
own support mechanisms to enable structured reflection, including clinical
supervision, development of supportive networks mentoring and coaching.
- Participate
in and undertake clinical supervision of colleagues on an individual or group
- Participate
in the Divisional appraisal System and demonstrate the achievement of agreed
personal objectives.
