Due
to the expansion of the PICS Community Pain/CFS pathway from Greater Nottingham
and Mid Nottinghamshire into Bassetlaw, an exciting opportunity has arisen for
an Advanced practitioner AHP / Nurse to support the multidisciplinary
team. We deliver an evidenced based
service tailored to the needs of Nottinghamshire ICB patients living with
persistent pain.
Our service stretches from Nottingham to Bassetlaw,
and we would take into consideration your home location when deciding your working
location. You will be part of a wider team of pain specialists, including physiotherapists,
occupational therapists, nurses, pharmacists, CBT therapists, psychologist, wellbeing
practitioners and pain consultants. You will be supported by our clinical lead,
Paula Banbury, who is on the PICS leadership team, and by Caroline Neal biopsychosocial
lead
Main duties of the job
KEY RESPONSIBILITIES
- We are looking for an individual with enhanced knowledge and skills or
wishing to develop these working in the specialist field of long-term pain and
fatigue management. The role involves
triage, biopsychosocial assessment and treatment offered both individually in
clinics and in groups, in community locations, of patients referred to the
service.
- Working outside your traditional scope of practice, as part
of the interdisciplinary team, using shared knowledge and skills of
physiotherapy, occupational therapy, nursing, and psychological interventions. You
will provide advice and education regarding changes in lifestyle, exercise,
pain mechanisms, medication, pacing, psychological impact of living with pain
and fatigue, and support the return to valued activities.
- Long term condition approaches such as Cognitive Behavioural
Therapy, Acceptance and Commitment Therapy (ACT), and Mindfulness underpin our therapeutic
offer to patients guiding them towards self-management, living well alongside
pain and fatigue.
- The role requires effective communication with patients, carers, the wider
MDT and relevant NHS and community service providers to ensure appropriate,
evidence based holistic care pathways for patients.

Practice
- To triage, assess and treat both individually and in groups, patients referred to the service. This will include the provision of advice and education regarding changes in lifestyle, exercise, pain mechanisms, medication, pacing and return to valued activities.
- To be able to identify and refer on appropriately those patients who may have complex and/or serious pathology (red flags).
- To work outside traditional scope of practice, using shared knowledge and skills of physiotherapy, occupational therapy, nursing, and psychological interventions.
- To liaise with primary and secondary care professionals regarding referrals to and from the service and be a clinical resource for advice on the management of persistent pain, ME/CFS and Long Covid fatigue.
- To recognise and make recommendations for treatment with different types of pain, e.g., nociceptive, and neuropathic pain.
- To establish collaborative working relationships with patients providing information and advice at all stages of the patient pathway. Communicating complex condition related information within scope of professional practice accounting for potential barriers to communication.
- To ensure effective systems are followed, integrating risk assessment and management and clinical governance to reduce clinical risk and promote patient safety.
