Job responsibilities
KEY RESPONSIBILITIES:
Within the Cardiopulmonary Rehabilitation (PR) aspect of the service
• The Cardiopulmonary Rehabilitation (PR) aspect of the service is based at community sites within Notts West. The role will require the A.P to support PR pre/post assessment, including recording of observations.
• In this role you will be expected to perform warmups and support patients within the exercise class, as well as progress exercises to achieve the desired outcome with the patient.
• To complete appropriate interventions following qualified clinician input and provide treatment for patients referred in for cardiopulmonary rehabilitation under the guidance of a Physiotherapist or RNS.
• Support clinical admin for the services (including but not limited to NACAP Audit input, report writing, inputting Assessment results/QOL indicators, developing patient information leaflets etc)
• To ensure that rooms are set up appropriately and checked as indicated and stock replenished and rotated as required, ensuring that internal cupboards are clean and stock is checked re expiry dates.
• To undertake weekly checks of the emergency equipment
Within Community Respiratory Aspect of the Service
• Respiratory nurse aspect of the service is supporting patients in their own home. The role will require the A.P to be proactive in delivering anticipatory and responsive care, promoting independence, healthy lifestyle, to improve the overall quality of life in the service users in their own home.
• With an ability to identifying risk or concerns with this cohort of patients and delegate appropriately to senior clinicians
• Initiate, organise and plan chronic disease management e.g. respiratory reviews, follow ups from designated caseload holder. Provide specific education to those patients relevant to their long-term condition management.
• To measure and record on approved templates, physiological signs - alerting a registered practitioner in an appropriate timeframe if physiological signs are outside normal parameters:
o Blood pressure
o Pulse rate and rhythm
o Temperature
o Oxygen saturations
o Height and weight (BMI)
• You will also be expected to perform standard clinical assessments, within your level of competency and supported by registered healthcare professionals, keeping accurate records for review as well as perform domiciliary visits at the request of senior colleagues.
• This service delivers specialist respiratory care to patients in the community to improve the self-management of their condition. We provide domiciliary visits and clinics
• Plan, develop and implement health promotion / education as appropriate e.g. smoking cessation, exercise and diet, and inhaler technique in line with own training and competences.
• Develop an awareness of Home Oxygen and support the team in the management of patients in the community using Home Oxygen.
• To target people who would otherwise face unnecessary prolonged hospital stays or inappropriate admission to acute in-patient care, long-term residential care, or continuing NHS in-patient care.
• To help maintain loan nebuliser service with checking nebuliser equipment, cleaning routine, stock of masks.
Communication / Utilising Information Systems
• Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.
• Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
• Communicate in an appropriate timeframe, any concerns about patients’ health status to a registered clinician.
• Document care and accurately record data using SystmOne.
• Contribute to the work of the practice team in by utilising the data from SystmOne and other information systems to improve care.
Delivering Quality Service
• Work with the nursing and practice team to assess effectiveness of care delivery, for example through assisting with audits, reviews and evaluation of care.
• Support and participate in shared learning across the practice and wider organisation.
Team Working
• Understand own role and scope in the organisation and identify how this may develop over time.
• Work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of working.
• Work effectively with others to clearly define values, direction and policies impacting upon care delivery. Participate in team activities that create opportunities to improve patient care.
Management of Risk
The post holder must comply with all PICS policies and procedures which aim to ensure safe practice and minimise risk. These include:
• Infection control / needle stick – Protecting themselves and others against infection risks. All staff regardless of whether clinical or not are expected to comply with current infection control policies and procedures and to report any problems with regard to this to the manager. All staff at the surgery should attend infection control training and updates as required.
• Health and safety / COSHH policy.
• Fire policy.
• Incident reporting policy.
