Job summary
We have an exciting opportunity for a Practice Nurse to join our dynamic team. We are looking for someone who is enthusiastic and motivated to provide high quality care. We are seeking a flexible, enthusiastic, reliable and hardworking individual to join our team at Whyburn Medical Practice. You will be managed and supported by our Lead Nurse on site and benefit from support from the wider PICS organisation.
You will have the skills, knowledge and experience required to fulfill the job description. Expertise in the management of long-term conditions is required. A non-medical prescribing qualification is desirable but not essential. We believe in the value of developing our staff and will work with you to identify learning opportunities to enable them to reach their full potential.
PICS is a GP owned organisation that provides a wide range of services in primary and community care with the aim of improving the health and wellbeing of all our patients and their families. These services are provided in a range of settings including peoples own homes and GP surgeries. Our focus is on delivering all care as close to home as possible.

Main duties of the job
KEY RESPONSIBILITIES
- To assess, plan, develop, implement, and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being.
- To implement and evaluate individual treatment plans for patients with a known long-term condition.
- To identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition.
KEY REQUIREMENTS
- Registered first level nurse
- Membership of a professional body (NMC)
- Evidence of recent CPD and training
