ABOUT THE ROLE:

We have an exciting opportunity for an experienced musculoskeletal physiotherapist with an interest in persistent pain to join our dynamic team. We are looking for someone who is enthusiastic about providing high quality care.

This is a permanent post, but we would consider someone seeking a secondment opportunity.

PICS are supporting our Extended Scope practitioners to develop their skills and knowledge in line with the national musculoskeletal framework for Advanced Physiotherapist Practice.

You will be based within PICS Community Pain Service. You will be part of a wider team including Pain Practitioners, Consultants Advanced Nurse Practitioners, CBT therapist, Psychologist, Pharmacists, Health and Wellbeing Practitioners and an administrative team. You will be supported by Paula Banbury, Clinical Lead, and Mags Wigram, Lead Extended Scope Practitioner, who are on the PICS leadership team.

Main duties of the job

KEY RESPONSIBILITIES:

  • To provide a senior specialised clinical role in the Nottinghamshire Interdisciplinary Community Pain / CFS Pathway
  • To provide services in community venues across Nottinghamshire CCG locality as required
  • To undertake clinical review / triage of referrals received in line with CCG policy
  • To undertake advanced clinical assessment of community pain referrals in an outpatient setting and formulate a management plan.
  • To decide on and take responsibility for the mutually agreed patient care plan.
  • To support the team in developing services which enable patients with pain to increase their activity levels within a community setting, and therefore improve their physical and mental health whilst living with and managing their pain

KEY REQUIREMENTS:

  • Minimum of 3 years musculoskeletal outpatient experience at AfC B6 level or above
  • Post-graduate certificate or masters level qualification in a relevant field
  • Highly developed specialist clinical reasoning skills
  • Excellent communication skills with patients and co-workers
  • In depth understanding of pain neurophysiology with experience of providing care for patients with persistent and complex pain
  • Access to a car for travel between clinics.

Our innovative community pain /CFS pathway delivers an evidenced based service tailored to the needs of Nottinghamshire patients with persistent pain. As an Extended Scope Physiotherapist, you will demonstrate enhanced knowledge and skills in the management of long term pain and its impact upon individuals. This will include training in and experience of advanced clinical assessment within a musculoskeletal and physiotherapeutic setting, particularly including patients with persistent pain. You will have excellent communication skills which you will use to assess patients' clinical, social and psychological needs, to liaise with the wider MDT and health community, and to deliver physiotherapeutic interventions appropriate for this challenging clinical field. You need to be an excellent team player, dynamic and forward thinking, with ambition for self-development and service development, and a desire for involvement in service improvement.

JOB PURPOSE:

  • To maintain professional knowledge and skills in expert practice in all aspects of pain management

Key Result Areas/Responsibilities:

  • To triage, assess and manage complex clinical pathways, demonstrating expert practice in all aspects of pain management.
  • To undertake a physiotherapy role that requires extending scope of practice and developing an advanced level of professional autonomy and accountability in the provision of such care.
  • To contribute to the development of the service generally, and within the context of persistent pain managing patients goals and expectations towards increasing activity levels to maintain physical and mental health, rather than towards curing them of pain.
  • To promote the purpose of physiotherapy for this complex group of patients, which is to increase patients' confidence to manage their pain, by developing their understanding of pain physiology and why pain persists, and by engaging them in self-management strategies.
  • To apply the evidence base supporting the importance of exercise and maintaining activity levels. To enable them to succeed, patients are guided to increase activities using graded and paced approaches. Occasional passive treatments e.g. manual therapy and injections, may have temporary role, but they are only used as a way of helping patients move forward towards self-management.
  • To develop opportunities within the community pain service where patients can increase their activity levels whilst managing their pain levels. These will as far as possible be through local community health and leisure amenities to ensure that activity is normalised and made part of daily life. This may include local leisure amenities and centres. It will involve close working with the pain MDT.
  • To work closely with the health and wellbeing practitioners and where necessary the CBT therapist and BPS practitioners to help individual patients with fear avoidance to increase their activity levels.
  • To develop and provide a specialist physiotherapy service one to one for a restricted number of patients within pain clinic settings, where their needs cannot be met by the community physiotherapy service, and when they need specific treatment which cannot be given in a group or leisure centre setting.
  • To act as a physiotherapy expert within the pain service, working towards advanced practice, by ensuring involvement in continual professional development, through case studies, reflective essay writing, reading professional journals and articles, reviewing national guidelines with regard to current best practice, attending appropriate courses and conferences, and incorporating best practice into protocols and guidelines.
  • To demonstrate clinical expertise, supporting the development of pain awareness and knowledge across the local health economy.
  • To provide training and education for professionals within the PICS MDT and also within the local health community with regards to management of persistent pain

Professional:

  • To adhere to the professional code of conduct and practice and relevant national standards at all times.
  • To provide clinical education, training and evaluation, and support and guidance of clinical team staff in relation to the provision of a physiotherapy interventions, in particular those related to helping patients to increase their quality of life and manage their pain through increasing activity levels. This will include competency training of relevant staff including other physiotherapists and the health and wellbeing practitioner.
  • To maintain, develop and disseminate specialist knowledge of evidence based practice, particularly that related to physiotherapeutic interventions.
  • To critically evaluate own work through the use of evidence based projects, audits and outcome measures.
  • To participate in the promotion and development of best practice, recommending and implementing changes to improve service delivery.
  • To continue to develop and implement evidence based approaches for the management of persistent pain, as part of an inter-disciplinary team, with particular reference to physiotherapeutic interventions.

  • To facilitate the communication of highly complex information regarding the case management of patients, both within the pain MDT, and also with professional colleagues across all areas of the health community, particularly GP practice staff in the CCG areas.
  • To use advanced communication skills to communicate with patients to gain consent for treatment within a care pathway. Highly sensitive and contentious information is regularly required to be communicated to patients after clinical and medical results are collated formulating specific pain management plans.
  • To be sensitive to the fact that barriers to understanding can be encountered with elderly patients, those without English as their first language, patients with disabilities and those with mental health difficulties such as anxiety, clinical depression or personality disorders. This can result in confrontational behaviour towards the practitioner, which needs to be addressed professionally and with compassion and understanding for the reasons behind this.
  • To show empathy and sensitivity in the communication of contentious information, for example weight loss advice, return to work recommendations to patients reluctant to do so and to those patients with a clinical diagnosis of poor prognosis.
  • To prioritise appropriately in unpredictable situations where clinical decisions need to be made urgently and to manage time and use the MDT where this may impact into time allocated towards other necessary tasks during clinic time.
