Job summary

As an experienced, registered Occupational Therapist, you will have experience of working within a multi-disciplinary team and across organisational boundaries to provide clinical assessments to patients living in care homes. You will prioritise the care home residents for review according to need based on identification by the MDT, risk stratification tools, clinical judgement, and care home advice.

You will be based within Arrow Health Primary Care Network (PCN) covering:

  • Westdale Lane Surgery, NG4 3JA
  • Daybrook Medical Practice, NG5 6HP
  • The Ivy Medical Group, NG14 5BG
  • Peacock Healthcare, NG4 1HQ
  • Plains View Surgery, NG3 5LB
  • Unity Surgery, NG3 6EU

Arrow Health PCN has a patient population in excess of 41,000. You will be part of a wider team of Occupational Therapists and supported by our Medical Director who is on the PICS leadership team.

Main duties of the job

KEY RESPONSIBILITIES

  • To make clinical decisions based on latest evidence and local/national policies and guidelines
  • To challenge differing opinions professionally and appropriately as a patient advocate
  • To develop and lead innovative concepts, models, methods, and practices to deliver new and improved primary care services to meet the needs of the practice population for those living in care homes and at home.

KEY REQUIREMENTS

  • BSC or MSc in Occupational Therapy under a training programme approved by the Royal College of Occupational Therapists
  • Registered Occupational Therapist on HCPC
  • Evidence of post graduate education

Job responsibilities

JOB PURPOSE

This is a key and pivotal role to support the delivery of the National Care Home Direct Enhanced Service (DES). The post holder will work across organisational boundaries, developing new ways of working to promote relationships between providers and statutory bodies and improve care for people living in long term residential care and across the PCN.

The post holder will develop and lead innovative concepts, models, methods, and practices to deliver new and improved primary care services to meet the needs of the practice population for those living in care homes and at home.

The post holder will prioritise care home residents for review according to need based on identification by the MDT, risk stratification tools, clinical judgement, and care home advice.

The post holder will ensure that all newly admitted residents and recently discharged from hospital residents will have a comprehensive assessment including mobility and a falls risk assessment. Many falls and fractures can be prevented and managed by well organised services and organisations working in partnership with the individual residents and there carers. The post holder will ensure that collaborative working across the therapy and nursing teams within Notts Health Care Trust is paramount.

As a Clinician within the PCN, the post holder will work with colleagues and clinical directors to continually develop local services.

KEY RESPONSIBILITIES

Clinical Duties

  • The post holder is responsible for ensuring that all newly admitted residents and those discharged from secondary care settings have a completed mobility and falls risk assessment and ensure outcomes of assessments are followed up by the most relevant Health Care Professional
  • The post holder will develop a robust care plan, make, and follow up referrals as required to reduce falls and to provide a clear action plan to manage any fall to reduce the need for hospital attendance.
  • The post holder is responsible for ensuring that the residents and care home managers receive assistance in accessing health care interventions by signposting to the most appropriate person to provide care
  • The post holder is responsible for consulting and collaborating with other Health Care professionals and specialists across both primary, community, secondary, social care, and the voluntary care sectors.
  • To take a significant role in the holistic assessment and treatment of patients in care homes with complex illnesses. To confidently consult with patients/families/carers to determine the needs of individuals and develop and review care plans to support care.

Professional Leadership and Development:

  • To be responsible for maintaining own competency to practice through Continuing Professional Development activities and to participate in the review and appraisal process.
  • To adhere to the individual Professional code of conduct and practice always.
  • To reflect on own practice individually and/or through regular clinical supervision.
