We have an exciting opportunity for a Senior Social Prescribing Link Worker to join our dynamic team. We are looking for someone who is enthusiastic about providing high quality care.

- The Valley Surgery, 81 Bramcote Lane, Chilwell
- Chilwell Meadows Surgery, Ranson Road, Chilwell
- Manor Surgery, Middle Street, Beeston
Eastwood neighbourhood:
- Newthorpe Medical Centre, Harvest Road, Eastwood
- Eastwood PCC, 11B Church Street, Eastwood
- Giltbrook Surgery, 492 Nottingham Road, Giltbrook
- Hama Medical Centre, 11A Nottingham Road, Kimberley
Stapleford neighbourhood:
- Saxon Cross Surgery, Church Street, Stapleford
- Hickings Lane Medical Centre, 120 Ryecroft Street, Stapleford
- Linden Medical Group, Church Street, Stapleford
You will be part of a wider team of Senior Social Prescribing Link Workers and Social Prescribing Link Workers, and supported by our Social Prescribing Manager who is on the PICS leadership team.
Main duties of the job
KEY RESPONSIBILITIES
- To lead and develop the Social prescribing team in the delivery of an effective and responsive support service.
- To support the identification and implementation of new ways of working to increase efficiency and effectiveness of the service.
- Maintain a high level of confidentiality in the teams work.
- To deal effectively with patient queries and escalate any complaints to the service to management.
- To promote a healthy and fair working environment for the Team.
KEY REQUIREMENTS
- NVQ Level 3 or equivalent
- Demonstrable commitment to professional and personal development
