ABOUT THE ROLE
We have an exciting opportunity for a PCN Care Co-ordinator to join our developing Care Coordinator Team. Working closely with Care Homes, General Practices and other services, you will provide coordination and administrative support to other members of the PCN. We are looking to expand our team to cover additional clinical areas with palliative care and safeguarding identified as priorities.
You will be based within Ashfield North Primary Care Network, with a patient population of approx. 51,000. The below details the location of the GP Practices in Ashfield North PCN. The role will require the successful candidate to attend the GP practices in the surrounding areas when required.

You will be part of a team of PCN Care Coordinators.
Please note that the pay range is £20,330 - £24,882, depending on experience, not the pay range listed above.
Main duties of the job
KEY RESPONSIBILITIES
- Coordinate Multi-Disciplinary Team meetings for people living in care homes, Safeguarding, or Palliative Care
- Liaise with GPs and other Health Care Professionals, also external colleagues in Care Homes, Health Visitors, and the Multi Agency Safeguarding Hub.
- Utilise Population Health Intelligence to proactively identify care home residents and work with the PCN team to enable the PCN team to deliver personalised care
- Utilise Population Health Intelligence tools where needed to support other patients in the PCN
- Ensure GP Practices data recording is accurate, and to use data to proactively search for case finding
- The ideal candidate will participate in an element of pre-emptive preparation with a view that MDTs will function as efficiently as possible.
KEY REQUIREMENTS
- Good standard of education GCSE or equivalent in English and Mathematics
- Experience of office procedures working at a high level as part of an administration team / within an administration role
- Understanding and able to deal with confidential and sensitive issues when liaising with team members / other professionals
