Job description

Job responsibilities

JOB SUMMARY:

As a First Contact Physiotherapist, you will work alongside the multi-disciplinary teams within the Primary Care Network. You will be a dedicated professional who shares the values of PICS and the NHS to deliver high quality primary care. You will clinically manage musculoskeletal conditions of your patients through clinical expertise working autonomously when assessing, diagnosing and treating patients with varying conditions.

YOUR PURPOSE:

To start your journey as a First Contact Practitioner - Physiotherapist (FCP), you will be an experienced clinician with a minimum of 3 years musculoskeletal experience and have a current HCPC registration status. A desire to work autonomously in the GP Primary Care setting and pursue further education is what drives you to join our team. If you thrive in an environment that challenges your skills and knowledge, and seek fantastic career development, we have a job for you.

You will be responsible for delivering frontline specialist musculoskeletal assessment, triage and care to a varied selection of NHS patients. This will involve both telephone triage and face-to-face assessments. Your role in the GP Practice will require close discussion with a multi-disciplinary team (MDT) to ensure the right care is delivered at the right time. You will be a self-motivated practitioner always striving to pursue excellence and the best care for your patient.

You will be responsible for referring patients into the MSK Hub for further triage and onward referral, as well as deciding if the patient needs support from other Primary Care colleagues. Taking the initiative and having ownership of a problem-solving approach to care are crucial behaviours for this post.

We have established FCP Physiotherapy Services for over a year and as such, we have a proven track record of effective and efficient service delivery. We have excellent feedback from our MDT team and patients. We are committed to quantifying our key performance indicators and share this information widely. In-service training is provided and on-going development of your professional skills is expected.

The FCP service will require you to perform frontline patient screening with direct referral from GP receptionists. From initial assessment, you will be responsible and accountable for assessing and managing MSK conditions to a high standard including onward referral to other health professionals, imaging or self-management. Injection therapy will also be utilised when appropriate. You will be an active participant in in-service training, FCP Roadmap developments and social media.

HEALTH EDUCATION ENGLAND FCP ROADMAP:

As part of this role you will be working toward fulfilling the requirements of the HEE FCP Roadmap. You will be allocated some time within your working week to complete this.

PICS is one of the largest employers of FCPs in England and has two Clinical Leads in post that are ensuring that its professionals are fulfilling the requirements of the Health Education England FCP Roadmap. We are committed to further the development of FCPs as specialist MSK professionals in primary care.

GP mentorship, support and supervision is integral to the post, as well as the full support of the Clinical Leads who are working closely with Health Education England and the East Midland CSP network. Fulfilment of registration on the HEE FCP Register is on-going across the Mid Notts region.

HOW YOUR ROLE FITS WITHIN OUR TEAM:

You will play a pivotal role in the existing FCP services, working in collaboration with the 6 Primary Care Networks (PCNs) in the Mid-Notts area: Mid-Nottinghamshire Integrated Care Partnership (ICP) - Nottingham & Nottinghamshire ICS - Nottingham & Nottinghamshire ICS (healthandcarenotts.co.uk).

You will work within a primary care setting, in busy GP Practices, as part of the wider community physiotherapy and MSK services. The FCP role is a subset of the Advanced Practice Physiotherapy network and as such, close collaboration with or partners is crucial.

KEY RESPONSIBILITIES:

  • To work as an autonomous specialist MSK practitioner in providing and delivering musculoskeletal physiotherapy assessments and treatment plans
  • To act independently to assess, analyse and provide a clinical diagnosis for individual patients, including those presenting with complex conditions, to determine their need for further MSK or non-MSK intervention. This should involve an understanding of when to refer to other services such as diagnostics, pathology, T&O, medication reviews, pain management, rheumatology, community physiotherapy and injection therapy
  • Write contemporaneous clinical notes using SystmOne, Arden's Templates and provide advice and support with additional electronic software (eg. PhysioTec, AccuRx etc) in line with professional standards
  • Fulfil mandatory training and those additional training requirements as demanded for the role
  • To understand each patients needs and expectations, and irrespective of their backgrounds and/or beliefs, to provide them with the quality of service to the standards the company expects
  • To demonstrate excellence in report writing, and evidence an ability to complete reports autonomously without the need for further support
  • To attend regular relevant courses both internally and externally, as well as keeping an up-to-date file of your CPD achievements, in line with CSP and HCPC guidelines. This is to be presented annually as part of the appraisal process
  • To identify and manage any risks to quality, such as workload, communication, and team working, with the support of your line manager and clinical mentor, and to advise on any issues regarding service delivery
  • To work as part of a team and liaise with other physiotherapists internally to ensure delivery of clinically effective care.
  • To communicate effectively with all stakeholders (including patients, clinical colleagues, administrative colleagues, receptionists, and external healthcare professionals etc) on challenging matters and in challenging situations in line with PICS Ltd & PCN procedures. Ensuring effective patient management, and working in collaboration with and/or referring to NHS colleagues in hospital and community settings
  • To fully understand the wider MSK service and local MSK pathways: across primary care, secondary care and the voluntary sector, including in depth knowledge of referral criteria and scope of services
  • To take an active role in engaging with external fellow physiotherapy professionals, working to promote PICS Ltd as a leading healthcare organisation, being aware of the referral pathways and actively suggesting and engaging with initiatives to help the business improve patient care
