Job summary
As an experienced, registered Dietician, you will be a key constituent of the Care Home MDT and will provide dietetic support to care home residents and training to staff. You will support the learning and education of healthcare professionals in the PCN and practices working closely with the Care Home MDT and Community teams.
You will be based within Arrow Health Primary Care Network (PCN) covering:
- Westdale Lane Surgery, NG4 3JA
- Daybrook Medical Practice, NG5 6HP
- The Ivy Medical Group, NG14 5BG
- Peacock Healthcare, NG4 1HQ
- Plains View Surgery, NG3 5LB
- Unity Surgery, NG3 6EU
Arrow Health PCN has a patient population in excess of 41,000. You will be part of a wider team of Dieticians and supported by our Medical Director who is on the PICS leadership team.
Main duties of the job
KEY RESPONSIBILITIES
- Provide an enhanced dietetic service to the frail patients, specifically those in care homes or complex chronic health conditions.
- Make dietetic advice readily available.
- Develop and deliver nutrition teaching programmes to care homes.
KEY REQUIREMENTS
- Bachelor of Science degree in Nutrition & Dietetics or equivalent
- Registered with the Health and Care Professions Council (HCPC)
- Evidence of continuing professional development
