ABOUT THE ROLE:

We have an exciting opportunity for a Clinical Care Home Nurse to join our dynamic team. The post holder will work across organisational boundaries, developing new ways of working to promote relationships between providers and statutory bodies and improve care for people living in long term residential care.

You will be based within Newark Primary Care Network with a patient population in excess of 76,300, covering:

  • Collingham Medical Centre, High Street, Collingham, Newark, Nottinghamshire, NG23 7LB

You will be part of a wider team of healthcare practitioners and supported by the PCN Business Manager and Clinical Director, who are on the PICS leadership team.

Main duties of the job

KEY RESPONSIBILITIES:

  • The post holder is responsible for contributing to a Multi-Professional team of HealthCare Professionals within the Care Homes team employed by Primary Integrated Community Services Ltd
  • To lead in the weekly home rounds and have the ability and knowledge to make decisions regards appropriateness of onward referrals and hold others to account for their delivery of actions.
  • To take a significant role in the holistic assessment and treatment of patients in care homes with complex illnesses. To confidently consult with patients/families/carers to determine the needs of individuals and develop and review care plans to support care.

KEY REQUIREMENTS:

  • Registered Practitioner - NMC or HCPC
  • Experience of working within a multi-disciplinary team and across organisational boundaries
  • Significant experience of working across different Health Care settings

JOB SUMMARY:

This is a key and pivotal role across Newark PCN, to support the delivery of the National Care Home Direct Enhanced Service (DES). The post holder will work across organisational boundaries, developing new ways of working to promote relationships between providers and statutory bodies and improve care for people living in long term residential care.

The post holder will contribute to the development and provision of the Enhanced Health in Care Homes service across the Newark Primary Care Network (PCN) locality by working closely with a Multi-Professional team to support the delivery of the weekly home round.

The post holder will develop and lead innovative concepts, models, methods and practices to deliver new and improved primary care services to meet the needs of the practice population.

The post holder will prioritise residents for review according to need based on identification by the MDT, risk stratification tools, clinical judgement and care home advice.

The post holder will be responsible for ensuring the consistency and appropriateness of staff required for attendance at the weekly home round

The post holder will work with colleagues to ensure that a full holistic review for all newly admitted residents and recently discharged from hospital residents and up to date care plans for residents are in place. The post holder will work to identify care needs both physical mental and care needs and refer independently to their appropriate colleagues.

The post holder will support the GPs and care homes in the delivery of alignment of care homes to practices.

SUMMARY OF DUTIES:

  • The post holder is responsible for contributing to a Multi-Professional team of HealthCare Professionals within the Care Homes team employed by Primary Integrated Community Services Ltd
  • The post holder is responsible for ensuring that the residents and care home managers within Newark PCN locality receive assistance in accessing health care interventions by signposting to the most appropriate person to provide care
  • The post holder is responsible for consulting and collaborating with other Health Care professionals and specialists across both primary, secondary, social care and the voluntary care sectors
  • To take a significant role in the holistic assessment and treatment of patients in care homes with complex illnesses. To confidently consult with patients/families/carers to determine the needs of individuals and develop and review care plans to support care.
  • To confidently make autonomous decisions following assessments and ensuring that information is collaboratively communicated back to GP colleagues in a timely and professional manner
  • To lead in the weekly home rounds and have the ability and knowledge to make decisions regards appropriateness of onward referrals and hold others to account for their delivery of actions.
  • Make recommendations regarding patient management in accordance with Local and National guidelines within the management of the older population.
  • To develop the evidence base for health needs led service planning, delivery and commissioning.
  • Work with GP leads and the Care Homes Lead in developing the strategic and operational management of the Enhanced Health in Care Homes Service.
  • To be responsible for participating in and implementing the overall strategy for development and delivery of the Enhanced Health in Care Homes service across Newark.
  • To establish and improve professional standards of care in the management of residents within care homes by developing partnerships with key stakeholders.
  • To support and assist the management with continuing professional development, training and peer appraisal of team members.
  • To develop / prescribe specialised programmes of care / care packages for all patients referred to the team, monitor and evaluate effectiveness of implementation. This includes developing training programmes and providing guidance to all clinicians both professional and non-professional across the health community.
  • Demonstrate a good understanding of clinical governance and risk management and apply to work effectively alerting any risks and benefits when developing services and practice in new areas.

  • Communications across the MDT is a critical element for this role. Information will be shared across providers, and the post holder will support the voice of the residents and care homes as equal members of the MDT.
  • Utilising and interpreting data, to support service improvement for care homes and primary care, as well as understanding the demands on urgent and emergency services to reduce avoidable activity where possible. Data and local intelligence will be utilised to develop and influence new ways of working to improve patient care, the resilience of primary care and system partners.
