Job responsibilities
DUTIES AND RESPONSIBILITIES:
CLINICAL
1. Work independently with minimal supervision.
2. To undertake nurse led clinics in the line with identified protocols and clinical guidance. Patients who will be on this register include:
- Patients with a specific elevated PSA, whose MRI is suggestive of Pirads 2 prostate (more in favour of benign prostate), who may need a rereferral to urology
- Patients with elevated PSA with MRI suggestive of Pirads 3 prostate, who will need referral if PSA density goes above 0.12.
- Patients on watchful waiting, if symptomatic or PSA above 20 then re-referred to hospital.
- Patients on watchful waiting, if symptomatic and PSA above 50 (if bone scan negative, again will be discharged to community)
- Patients with prostate cancer who have had radical radiotherapy, whose PSA is < 1 will be discharged into community, to be referred if PSA above 2.5 or symptomatic to oncology.
- Patients with prostate cancer who are 2 years post radical prostatectomy with PSA<0.04, will be discharged with community follow up. To be referred if PSA above 0.1 or symptomatic.
- Patients with elevated PSA, with MRI pirads 4 and above, biopsy negative both TRUS, template and PSA stable but may need to be re-referred if PSA crosses a nadir.
3. Identify overt and covert patient problems through assessment, communicate the findings of the consultation to the patient and primary carer(s) and plan action. This might include: education of the patient and primary carer(s) recommendations for change(s) in therapeutic regimes, requests for more data from patient, initiation of specialist care techniques. Evaluate, with the patient and primary carers, the effectiveness of recommended interventions.
4. Advise and support the needs of patients and their carers, acting as their advocate to promote and facilitate quality and individualised care, providing written information as necessary.
5. To liaise with the medical team to report any significant clinical changes reported by patients and facilitate any recommendations following medical discussion.
6. To promote and maintain effective communication.
7. To demonstrate specialist skills and knowledge through expert nursing practice.
8. To act as a resource for the specific skills and care for patients, carers and other Multidisciplinary team members across the Primary Care Network.
9. To oversee a high standard of patient care based on the assessment, development, implementation and evaluation of programmes of care, involving the patient, those close to them and the care provided.
10. In collaboration with the Multidisciplinary team, to develop specific written information for patients and carers.
11. Establish and maintain accurate documentation of the assessment and management of urological disorders in accordance with the UKCC standards for Records and Record Keeping.
MANAGERIAL DUTIES:
1. To attend and actively participate in the multidisciplinary team meeting and treatment planning/referral decisions where required.
2. To assess the quality of care given by the evaluation of patient service, effecting change where necessary and demonstrating patient user involvement when appropriate.
3. To liaise with the Clinical Lead on issues of service management and development for the Primary Care Network.
4. To liaise closely with the Clinical Lead, maximising the use of available resources to benefit patient care and ensuring that any changes in practice that impact budgetary resources are considered fully.
5. To promote/manage the efficient and effective use of supplies, provisions and equipment within the designated budget.
The post-holder will provide high quality specialist nursing care to patients with urological conditions. The post-holder will provide support and advice to patients and their families as well as acting as a resource to other professionals.
EFFORT AND ENVIRONMENT
PHYSICAL EFFORT
Frequent long periods of time spent holding telephone clinics and writing up patient related information within agreed time limits. There is a need for speed and accuracy. This is a continuous requirement throughout each day.
MENTAL EFFORT
Prolonged periods of concentration are required frequently throughout each shift when prioritising caseload. This can often involve changing from one task to another and back to original task with several interruptions in between.
Prioritising work is essential. The post holder is required to work under pressure and meet deadlines.
Attention to detail required when updating SystmOne following assessments.
Concentration on reading and writing reports is necessary.
Concentration for periods of time when working and discussing health issues with patients, families and carers.
EMOTIONAL EFFORT
Some patients will have complex needs. Some will have problems that can cause stress for the post holder.
The post holder can also be faced with distressing circumstances and emotionally demanding situations, which will come about occasionally when dealing clients and their families. There can be frustration when patients do not accept or action health plans.
WORKING CONDITIONS
This role will be based in a GP practice.
Occasionally the post holder may be exposed to aggressive verbal/physical aggression from patients, relatives and other health and social care professionals over the telephone or face to face.
SUPPLEMENTARY DUTIES AND RESPONSIBILITIES
MOBILITY
Employees may be required to work at any of the other sites within the organisation subject to consultation. The organisation is in a period of rapid change due to developments and rationalisation of services. This will lead to modification of structures and job descriptions. The post holder will be expected to co-operate with changes subject to consultation, at any time throughout the duration of their contract
