Job summary

Symphony Health Care Services is looking to recruit a Medicines Management / Pharmacy Technician, to join our Medicines Management Team, based at South Petherton Medical Centre.

You will work alongside a team of pharmacists, pharmacy technicians, prescription clerks across other practices in the PCN (Martock Surgery, Hamdon Medical Centre, Buttercross Health Centre in Somerton, Ilchester Surgery and Crewkerne Health Centre).

You will assist with medicines reconciliation, support drug monitoring work, and answering medication queries from patients and community pharmacies, as well as supporting the practice with all aspects of medication safety. You will liaise with GPs, practice staff, community pharmacies and secondary care colleagues, and have direct contact with patients and carers.

You will need to demonstrate the ability to use your initiative, have a positive attitude, a can-do approach towards problem solving, and be a good team player.

This is a permanent, full time vacancy of 37.5 hours per week. Job share opportunities will be considered - please state in your application.

The salary for this role is between £29,473 and £34,382 per annum, plus the option to join the NHS pension, plus many other employee benefits.

Main duties of the job

This role is fundamental in supporting the South Somerset West PCN in providing safe, effective and efficient systems for medicines reconciliation on transfer of care, and to support the repeat prescribing systems.

You will help patients to get the best from their medicines by supporting systems to improve the continuity of care, supporting improvements to streamline the repeat prescribing processes, minimising clinical risk and reducing waste. You will also be enrolled on and actively participate in a recognised post-registration pharmacy technician educational pathway.

You will be based at South Petherton Medical Centre but may be required to work across our other medicines management locations / pharmacies as and when required, to meet the needs of the service.
