Job summary

Symphony Healthcare Services has an opportunity for 2 full-time enthusiastic and forward thinking Clinical Pharmacists to join our established and growing teams in providing meaningful, patient centred support in general practice.

You will work across 2 main sites - Martock Surgery and South Petherton Surgery in Somerset, but also be part of Symphony South group of PCN practices, which includes Hamdon Medical Centre, Crewkerne Health Centre, Buttercross Health Centre in Somerton and Ilchester Surgery.

The salary range is £50,562 to £60,570 per annum and starting salaries will be offered based on previous experience. You also have the option to join the NHS pension scheme and access many other employee benefits.

We have 2 x 37.5 hour permanent posts vacant but part time hours or job share opportunities will also be considered - please state in your application your preference.

Main duties of the job

To provide Pharmacy services within primary care based within the South Somerset West Primary Care Network.

To support, promote and implement high quality, evidence-based, cost-effective and safe use of medicines. This includes:

Ppatient medication reviews

Helping the practices within the PCN achieve prescribing targets

Supporting the implementation of national and local guidance in order to improve patient care.

Processing prescription requests, queries and prescribing efficiencies

Ensuring safe prescribing practice and completing audits

Youwill need to demonstrate:

• Good IT skills with the ability to obtain and analyse complex technical information
• Recognise priorities when problem solving and identify deviations from normal patterns
• Work effectively independently and as a team member
• Demonstrate accountability for delivering professional expertise and direct service provision.
