Job summary

We have an exciting opportunity for a Pharmacy Technician to join our Medicines Management Team based across Burnham Medical Centre and Highbridge Medical Centre. You will work alongside a team of pharmacists, pharmacy technicians, prescription clerks.

You will assist with medicines reconciliation, support drug monitoring work, and answering medication queries from patients and community pharmacies, as well as supporting the practice with all aspects of medication safety. You will liaise with GPs, practice staff, community pharmacies and secondary care colleagues, and have direct contact with patients and carers.

You will need to demonstrate the ability to use your initiative, have a positive attitude, a can-do approach towards problem solving, and be a good team player. Full training will be given.

Rate of pay is £14.55 to £16.97depending on experience with a NHS or NEST pension offered.

Main duties of the job

MAIN DUTIES AND RESPONSIBILITIES

  • To carry out medicines reconciliation in accordance with practice protocols and to update patients medical records and prescriptions.
  • To carry out medication switches, dose optimisation and medicines synchronisation to ensure safe and cost-effective prescribing.
  • To initiate repeat prescribing for appropriate patients
  • To provide advice to GPs, staff, and patients in changes to prescribing in order to support the improvement of prescribing safety, quality and cost effectiveness.
  • To participate in medication audits and QOF quality improvement projects
  • To respond to medication queries from patients and staff in a professional manner referring to the appropriate GP in accordance with practice protocols
  • To support care homes in maintaining effective medicines management and reduce waste.
  • To carry out domiciliary visits where required to support patients use of medication.
  • To advise on the sourcing and safe management of medicines as appropriate
  • To action drug alerts
  • To act as a link between practice and local community and hospital pharmacy services
  • To provide information on KPIs to show benefits of the post.
