Job summary
We are looking for an enthusiastic and committed Advanced Nurse Practitioner to join our established and highly motivated team at Hamdon Medical Centre. This post available is working 18.75 hours per week, Monday to Friday.
Salary will be dependent on experience. Hours and working patterns to be discussed at interview but hours can be flexible based on the successful candidate.
Main duties of the job
The post holder will be an experienced Nurse who, acting
within their professional boundaries, will provide care for the presenting
patient from initial history taking, clinical assessment, diagnosis, treatment
and evaluation of patient care.
The post will holder will be able to demonstrate the
ability to make differential diagnosis using decision making and problem
solving skills whilst making professional autonomous decisions for which they
are accountable. They will possess the ability to work independently and to
work collaboratively as part of a multi-disciplinary team with other health
care professionals and disciplines, supporting the delivery of policy and
procedures. NMC requirements for advanced practice must be met.

Hamdon Medical Centre is a well-established, friendly,
patient centred practice with an in-house part owned pharmacy. There has been a
GP surgery in the village of Stoke Sub-Hamdon since 1865. Hamdon Medical Centre
was a new purpose built surgery in 1990 and was designed for the comfort and
safety of all patients, particularly those with mobility difficulties.The practice
provides a wide range of services to around 5,500 patients and the 2 employed
GPs are supported by a multidisciplinary team of health coaches, nurses and
administration staff.
The team are committed to a high standard of general
practice, working in a integrated way across primary and secondary care and
working closely with community staff.
