Job summary

The successful applicant will provide expert care, advice and support to patients who have recently undergone stoma forming surgery within the Bradford community.

This position will be working as part of a small Community Team led and supported by a team leader

Main duties of the job

Areas of Responsibility

  • To comply and work within the NMC The Code (2008)
  • To provide and develop a community nursing service, ensuring patients receive the highest standards of care in line Salts Medilink working in partnership pathway.
  • Responsibility for a defined patient caseload, including setting standards of care, assessment of care needs, development, implementation and evaluation of programmes of care.
  • Provide education for professionals, patients and carers. Act as a resource, providing optimum care for patients who are to undergo or who have undergone stoma surgery.
  • Provide education to patients and carers on correct usage and ordering of stoma supplies.
  • To have extensive knowledge on the diseases, treatments and best nursing practice relating to colorectal diseases and those bladder conditions requiring stoma formation.
  • Be accountable for own professional actions as is not directly supervised.
  • Be responsible for assessing, planning, implementation and evaluation of evidence-based packages of care for patients, using advanced clinical knowledge and skills.
  • To support other team members as required in managing specialist caseloads.

You will be working as part of a small team supported by a team leader

Experience of working within a Stoma Care environment desirable but not essential as further training will be given.

Registration on Part 1 of NMC Register.

3 years post registration experience with relevant experience in stoma care nursing

Demonstrated evidence of personal professional development

Effective communication skills

Ability to work alone and as part of a team

Membership of RCN

Full UK driving license
