We have an opportunity for a Senior Clinical Pharmacist to join their team of staff in the Pharmacy department.

Previous oncology experience is desirable but not essential. We welcome applications from pharmacists with a strong clinical background who are enthusiastic to develop their skills in oncology. Comprehensive training and mentorship will be provided by our experienced team, ensuring full support as you transition into this specialist role. This is a Full time role for 37.5 hours a week. The role holder will be required to cover a flexible shift pattern.

Main duties of the job

Provide expert clinical pharmacy input as part of the multidisciplinary team, conducting medicines reconciliation, clinical screening, and therapeutic drug monitoring.

Optimise medicines use through evidence-based interventions, ensuring the safe and effective management of long-term and acute conditions.

Support prescribing decisions through clinical assessment, risk evaluation, and formulary guidance.

Lead on implementation of medicines optimisation strategies, ensuring prescribing is cost-effective, safe, and aligned with national/local guidelines.

Contribute to the development and review of clinical guidelines, protocols, and policies.

Monitor and report on medicines usage, compliance, and safety incidents, participating in root cause analysis and quality improvement.

Collaborative Working and Clinical Leadership

Attend ward rounds, clinical meetings, and governance forums, offering proactive input into patient care plans.

Represent pharmacy in multidisciplinary quality initiatives and clinical service reviews.

Policy, Safety, and Compliance
