Job summary

This is an opportunity to work as part of a charity committed to our vision of enabling everyone in our communities to achieve better mental health. TOG Mind is a fast-paced charity delivering a wide variety of mental health and emotional wellbeing services. In your role as a Community Child & Adolescent Mental Health Service (CAMHS) Navigator you will work as part of the Children, Young People and Families Directorate and will be responsible for supporting children and young people aged 5-18 transition from statutory mental and emotional wellbeing services, into community support to help aid their recovery, and sustain outcomes following completion of work with CAMHS.

Main duties of the job

You will be required to work closely with local CAMHS services, including being co-located within the CAMHS service weekly and to collaborate with lead provider Early Break to ensure seamless access to support.

You will carry a caseload of clients who you provide support to, at times and locations that support access and meet the needs of the client. You will provide a range of therapeutic interventions to support the continued self development and achievement of young peoples self defined goals

You will be familiar with the local community and emotional health and well-being services that are available for children and young people to access.

You will be responsible for the development of relationships with partners from local statutory and community-based services, and ensure that the young people you are supporting, receive a package of care and support that enables them to thrive in their local community.

Work as an integral part of the Children, Young people and Families Directorate to deliver services directly to members of our community.

Provide support to people within the boundaries of our services, identify any additional support and sensitively help them to access other services as needed.
