Function Specific
Responsibilities
The project support officer role is vital to the
ongoing development and sustainability of My Health Huddersfield (MHH) GP
Federation through contribution to engagement, improvement and implementation
of new services.
The post holder will support effective
partnership working with colleagues across GP practices and Greater
Huddersfield Clinical Commissioning Group (CCG), as well as external health and
social care partners and third sector organisations.
The role includes the following
responsibilities:
1.0 Clerical & Admin
Keeping an overview of
MHH Directors diaries and providing support where necessary.
Ensure minutes for the MHH Directors meetings
and MHH Shareholders meetings are circulated in a timely manner and any actions
are completed within the agreed timescales.
Organise meetings,
workshops and training sessions and provide appropriate support.
This will involve
making all required arrangements and may include coordinating internal/external
speakers, booking venues and facilitation. This will include, but isnt limited
to the following meetings:
o MHH Director
o MHH Shareholders
o Primary
Care Network
o CCG membership & engagement
o Individual practice meetings.
o Virtual
lunchtime training sessions
2.0 Comms & Engagement
The post holder will
work as part of the MHH team, ensuring that the co-ordination and development
of projects/services is integrated and information is accessible.
Developing and
maintaining links with the CCG to ensure the most effective and efficient
communications to GP practices.
Developing a timely
review process to ensure GP practice, MHH director/staff and CCG feedback has
been considered and making suggestions for future ways of working.
Lead on the development of the monthly MHH newsletter and support the maintenance of MHH social media
channels .
Supporting engagement
through specific meeting responsibilities, but also through attendance at GP practices
to assist with the implementation of new services and reporting mechanisms.
Contributing to the
continuous review of services, including elements of patient engagement. Ensuring, where appropriate, that patients
are engaged and supported to be at the centre of the review process
3.0 Project Work
Lead on the development and maintenance of the
MHH S1 unit including rota management and reporting.
Working to a specified brief for
projects/services developed and delivered by MHH. This will include the development of:
mobilisation plans; reports for GP practice clinical systems: step-by-step
guides; monitoring & evaluation protocols and data analysis.
Liaising with key stakeholders, including
organising and planning task and finish groups/meetings to progress project
work and providing regular updates as identified by the MHH governance
structure.
Aligned to the implementation and development of
documents for projects/services, the post holder should use appropriate
tools/methodologies to ensure compliance with agreed outcomes, processes,
priorities and targets.
Set up robust administration systems to monitor
MHH/practice activity and achievement of contracting and monitoring outcomes as
required for each project/service provided.
This will include managing data bases in order to provide regular
reliable information and communicating with GP practices and partner
organisations on a regular basis.
4.0 Data Analysis and Reporting
The post holder will
contribute to the development of key reports, briefs and presentations as
required by MHH, with a specific responsibility for the collation and analysis
of data.
The post holder,
therefore, must be proactive in terms of collating and populating data and
highlighting any potential issues or risks in a timely manner.

deemed within the expectations of your position.
B. Key Responsibilities
The post holder must be able to:
Work on their own
initiative, taking advice from manager/MHH directors as required, recognising
their work will be assessed and embraces the positive aspects of this.
Support MHH to
deliver against agreed metrics, demonstrating improvement where appropriate.
Provide timely
advice, complex or sensitive information, or support to internal and external
stakeholders, whilst ensuring adherence to policies, legislation, governance
requirements and best practice guidance.
Produce regular
timely reports in the most appropriate way
Gain cooperation and agreement from MHH shareholders and wider
stakeholders, using effective communication skills. This will include motivation and/or
negotiation skills where appropriate
Propose
changes/improvements to projects/services being delivered/led by MHH.
Handle information
relating to projects and MHH shareholders which may be sensitive, complex or
confidential and appropriately record, transfer and/or coordinate such
information in accordance with the Data Protection Act.
Make judgements on a
range of issues/queries/questions relating to MHH using own expertise and
within own responsibility level.
Plan and manage own
workload to meet required objectives, including regular updates to
manager/MHH directors.
Plan and coordinate meetings, programmes or complex events involving
Directors, managers, shareholders and other attendees from external
organisation. This may require
adjustment of plans and delivery in agreed timeframes.
Manage any allocated
financial responsibility this may include ordering of stock and supplies
and processing/management of invoices.
This must be done within financial rules.
Produce work using
the appropriate computer, keyboard (hardware) and software skills.
