Job summary
We are seeking an experienced Practice Nurse with long term condition management to join our team at Norwich Health Centre, Rouen Road, Norwich to lead and develop high quality patient care to meet holistic health care needs of the practice population, to work as part of a multi-disciplinary team delivering care.
Main duties of the job
To work as part of a multi-disciplinary team to ensure high quality and consistent patient outcomes and experience for our registered patients.
The successful candidate will ideally hold a qualification in Long Term Condition management.
Management of chronic diseases from diagnosis, monitoring and development of individual care plans, agreeing where appropriate with the patient and other health care professionals.

Norwich Health Centre is a CQC rated 'Good' practice with approximately 11,000 patients, situated in the heart of Norwich. Norwich Health Centre is led by Norfolk Primary Care working alongside the Practice Management team.
Our clinical team is supported by salaried GP's, and Advanced Nurse Practitioner, 3 Clinical Pharmacists, 2 Practice Nurses and 3 Health Care Assistants.
We also have a large non-clinical administrative team.

- Participate in shared learning across the practice and wider organisation and assess the impact of policy implementation on care delivery.

- GCSE Grade A-C (or equivalent) in English and Maths.
Desirable
- Qualification in Diabetes/Asthma/COPD. Non-medical prescribing qualification.
- Accredited long term condition modules such as Hypertension, Coronary Heart Disease, Chronic Kidney disease.
- Cervical cytology sample takers registration.

- Knowledge and understanding of general responsibilities to patient information and record keeping.
