Job summary
NN4, a Primary Care Network in North Norfolk, are looking for an experienced and motivated Health and Wellbeing Coach to join their multidisciplinary Mental Health Team.
The post holder could be expected to work across a number of surgeries within the PCN, which spans across Acle Medical Partnership, Brundall Medical Partnership, Blofield Surgery, Hoveton & Wroxham Medical Centre, Staithe Surgery, Ludham & Stalham Green Surgery.
The post holder would be an integral part of the general practice teams, working in a patient facing role for the benefit of patients, providing and maintaining a high standard of care. They will support patients to achieve their personalised mental health objectives whilst providing assistance and advice to other members of the primary care team in managing the mental wellbeing of the practice population.
Main duties of the job
As part of this multi-disciplinary team the post holder will work in a patient facing role to provide personalised support to individuals, their families, and carers to empower them to manage their own mental health and live independently. The role will also provide assistance and advice to the GPs and other members of the primary healthcare team in managing the wellbeing of the practice population. You will need to have the ability to influence and communicate effectively with clinical colleagues, patients and support staff.
Key role responsibilities include:
To engage patients in coaching and to motivate patients through multiple sessions to identify their needs, set goals, and supporting patients to achieve their personalised mental health objectives.
Provide interventions such as self-management education and peer support.
Supporting personal choice and positive risk taking while ensuring that patients understand the accountability of their own actions and decisions, thus encouraging the proactive prevention of further illnesses.
Working in partnership with the other social prescribing services to connect patients to community-based activities or support charities which enable them to take increased control of their mental health and wellbeing.
