Main duties of the job

Primary Care Support Team (PCST)

The PCST is supported by a Service and Clinical Lead. The successful candidate will work as part of the Primary Care Support Team Comprising of GPs, Advanced Nurse Practitioners, qualified nurses with a prescribing qualification (or working towards), and Advanced Paramedics working across North Norfolk and Great Yarmouth and Waveney. The team provide additional Primary Care support to that currently being provided by individual practices, Care Homes and Housebound patients.

Another exciting element to the PCST Role is the GP Front Door Streaming service at the Norfolk and Norwich University Hospital, James Paget University Hospital and The Queen Elizabeth Hospital streaming patients who can be assessed and treated by primary care teams at the Emergency Department (ED) front door.

North Norfolk Primary Care (NNPC) is an established Primary Care provider, owned by the North Norfolk GP Practices. Our aim is to be a key provider of Primary Care services in the local and wider system working with the local GP practices and secondary care providers.

Job description

Job responsibilities

This exciting role will provide the post holder with the opportunity to work with an award winning forward thinking innovative company. You will be an experienced Practitioner (which incorporates being an Independent Prescriber), or a suitably experienced Registered Nurse/ Paramedic with a prescribing qualification who, acting within their professional boundaries and competencies, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, referral, evaluation, and follow up of care.

They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for the patients within Primary Care. The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the key stakeholders to meet the needs of patients in supporting the delivery of policy and procedures and providing nurse leadership if/as required.

In order to work at this level, NMC requirements for Advanced Practice must be met.

Key Responsibilities:

Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly.

Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).

Refer patients for investigation where appropriate.

Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan.

Work with patients in order to support compliance with and adherence to prescribed treatments.

Ensure provision of continuity of care including follow up appropriately.

Take guidance and instruction from the GP, when and if required.

Provide information and advice on prescribed or over-the-counter medication on medication regimes, side-effects, and interactions.

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

Refer patients to other health care professionals as appropriate for clinical needs and in accordance with local framework/guidelines if indicated.

Meet the needs of patients presenting for opportunistic wound care.

Be able to undertake medication reviews for individuals on any type of medications within an agreed skill level.

Verification of expected death.

End of life - to support advanced care planning.

Be able to undertake general telephone triage/first contact assessment.
