We have an exciting
opportunity to join an NHS Parliamentary award winning team for Excellence in
Primary Care.
North Norfolk Primary Care (NNPC) is an established primary
care at scale provider, owned by the
19 practices in North Norfolk. Our aim is to be a key provider of primary care services
in the local system working
with local GP practices. NNPC is rated GOOD
by CQC
NNPC has commissioned an enhanced care
home team
comprising advanced
nurse practitioners, qualified
nurses and advanced paramedics working across North Norfolk. The staff will provide
additional support to that
currently being provided by individual practices.
Supported by a Service Lead, the
clinical staff will be responsible for working with all care homes and practices across North
Norfolk, focusing on proactive support to care homes
and patients residing at home.
Main duties of the job
This
exciting role will provide the post holder with the opportunity to work with an
award winning team. You will be an
experienced Advanced Nurse Practitioner
(which incorporates being an Independent Prescriber), or a suitably experienced
Registered Nurse with a prescribing qualification who, acting within
their professional boundaries and competencies, will provide care for the presenting
patient from initial history taking, clinical assessment, diagnosis, treatment, referral, evaluation, and
follow up of care.
They will demonstrate safe, clinical
decision-making and
expert care, including assessment and diagnostic skills,
for the patients within Primary
Care. The post holder will demonstrate critical thinking in the clinical decision-making
process. They will work collaboratively with the care homes and GP Practices to meet the needs
of patients in supporting the delivery of policy and procedures, and
providing nurse leadership if/as required.
