Support Worker - Vulnerable Adult Service

Job summary

To work as part of a multidisciplinary Vulnerable Adult Service (VAS) team based in Norwich at the Inclusion Health hub. The post holder will provide both in reach and outreach support for vulnerable and marginalised adults with severe and multiple disadvantages who may be homeless, living in hostels or in temporary accommodation.

Support will be provided to individuals to register at the primary health care service at the inclusion health hub at under one roof and to further access to a range of health care services including but not limited to appointments in respect of their health and care needs.

The post holder will in addition support Vulnerable patients referred from Norwich Inclusion Health Practices as part of the Vulnerable Service model in respect of their health care needs The post holder will support the ambitions of Norwich PCN to address health inequalities by providing support services for marginalised groups across Norwich PCN, you will work collaboratively with a wide range of statutory, voluntary and charitable organisation to be able to offer a holistic approach to improve access to health care.

Main duties of the job

The post holder will actively engage with people who use the service on a one-one basis, providing a comprehensive support package that is sustainable, recovery focused, and goal orientated.

  • The post holder will provide an outreach service and will work at both the U1R base and a range of appropriate outreach venues.
  • The post holder will support, and coordinate patients access to hospital and other appointments in respect of their health and care needs.
  • The post holder will be required to chaperone where needed during GP, Health and Health Care professional appointments as directed by the clinical team and under the standard operating procedure, following training and enhanced DBS checks.
  • The post holder will undertake the guidance of the clinical team and under the standard operation procedure urine drug screening.
  • The post holder will carry out street outreach as required in collaboration with partners such as the Pathways Norwich service and support people to access the VAS at the hub or the Inclusion health practice they are registered at.
  • The post holder will attend and take an active part at weekly multidisciplinary team meetings and follow up actions and plans of care for patients.
  • The post holder will work closely with the VAS Integrated care coordinator to plan support needs and coordinate them. This will be in adherence to the guidance within the VAS standard operating procedure which requires support for patients to transition to the tiered levels of the service.
