Job summary
The Frailty Care Co-ordinator will have a key role in
working with the community services team and wider primary care team to ensure
frail patients in the community receive appropriate interventions to support
them to live as independently as possible. This will involve supporting the
administrative co-ordination of a range of clinicians and representing the team
at daily frailty meetings. The role will
include data collection, reporting and liaising directly with clinicians to
inform workloads.
Main duties of the job
The post-holder will, under the oversight of the PML
clinical practitioner and reporting to the Operations Support Manager be responsible for the
following:

Daily attendance at the frailty multidisciplinary team meeting

Attend weekly frailty MDT meeting
-
Assist with the coordination of referrals for
our community teams.

Allocate workload according to activity

Data collection for submission to internal
reports and national databases

Principal Duties under direction of
the daily clinical practitioner and supervised by the Operations Support
Manager
Administration Duties:
- Be the
co-ordinator of planned activity for the management of frail patients. Ensuring that healthcare professionals focus
their resources on those in most need, co-ordinating the right intervention for
each patient and ensuring that records are shared appropriately.
- Use data to
monitor and track activity to enable outcome reporting. Using this information to work on a daily and
weekly basis to inform healthcare professionals to inform their caseloads.
- Attend the daily
frailty multidisciplinary team meeting to represent the team and ensure a
coordinated approach to the workload.
- Ensure that
information from the MDT meeting is collated and captured accurately to inform
decision making, clinical assessments and enable reporting.

Take telephone messages and relay to the
relevant person/team in a prompt, efficient manner.
- Liaise
with General Practitioners, Practice Managers and other staff as appropriate. Help people to
manage their needs, answering their queries and supporting them to make
appointments.
- Support people to
take up training and employment, and to access appropriate benefits where
eligible.
