Job summary
The Site Clinical Lead is responsible for the delivery
of high quality clinical services on site, ensuring agreed standards are met.
The Site Clinical Lead
Has delegated responsibility for
clinical governance, contract and QOF performance for the site.
Will line-manage the clinical team
and ensure sound leadership on the site within the values of Salford Primary
Care Together.
Will work closely with the Senior
Leadership Team and GP Clinical Lead to contribute to service improvement;
coordination of activity across sites and the consistent implementation of
agreed policy.
Is also a Salaried GP and so has
the role and responsibilities of a Salaried GP.
Utilising in-depth Primary Care and clinical
knowledge, the Site Clinical Leads will be able to translate regulatory
developments into clinical service delivery designed to meet the needs of the
patients and contractual requirements. This includes: adhering to the GMC
standards for good medical practice, contributing to the effective management
of the practice, leading by example, maintaining a positive, collaborative
working relationship with fellow Site Clinical Leads and the multidisciplinary
team.
As part of the leadership and management of SPCT, the
Site Clinical Lead is responsible for enabling and supporting their team(s) to
meet the goals and outcomes so everyone can contribute to improving the
provision of care in Salford.
Main duties of the job
Patient Care
Undertakes
duties and responsibilities associated with a GP working within primary care,
including a full range of medical services as defined in the core PMS contract.
Ensure
patients are involved in the planning and improvement of services, including
attendance at Patient Forums
In
accordance with the practice timetable, work plan and appointment templates,
the post-holder will make him/her-self available to undertake a variety of
duties including surgery consultations, telephone consultations and queries,
visiting patients at home, checking and signing repeat prescriptions and
dealing with queries, results, paperwork and correspondence in a timely
fashion.
Making
professional, autonomous decisions in relation to presenting problems, whether
self-referred or referred from other health care workers within the
organisation.
Assessing
the health care needs of patients with undifferentiated and undiagnosed
problems.
Screening
patients for disease risk factors and early signs of illness.
In
consultation with patients and in line with current practice disease management
protocols, developing care plans for health.
