Job summary

We have an exciting vacancy for experienced Practice Nurses to join our bank working across our Enhanced Access Services delivered on behalf of Eccles and Irlam PCN at our clinical hub.

As this is a bank role this will suit an individual who wants flexibility of shifts and can cover clinics outside of core hours including the evenings and weekends.

Main duties of the job

The main duties of this role will include;

  • Cytology Clinics
  • Chronic Disease Management
  • Immunisations

Salford Primary Care Together is a community interest company with all GP practices in Salford as its members. Our aim is to work with our members and other partners to ensure general practice is at the heart of joined up care in Salford.

The Extended Primary Integrated Care (EPiC) Team is ran by Salford Primary Care Together to bring together primary and urgent care services through a central hub so that patients can be directed to most appropriate level of care they require.

Job description

Job responsibilities

Patient Services

  • To undertake nursing and therapy tasks including cytology, chronic disease management, health checks, vaccinations, assessment of risk factors, and lifestyle advice.
  • Manage the care of patients with long term conditions. Assess, plan, implement and evaluate treatment plans for patients with long term conditions (or at risk of developing one) that meet identified needs and promote health and well-being.
  • Assess the needs of patients, providing appropriate care/treatment in conjunction with the GP, or independently, refer to colleagues or other agencies as appropriate. Ensure that when referrals are made for patients to see other medical service providers, these are kept within local guidelines, and that adequate information is given within the referral to enable further assessment.
  • Making every contact count. Front line staff are in an ideal position to offer support and advice on how to improve health and wellbeing. Staff will be given training and support to help them signpost people to other services.
  • Maintain accurate and contemporaneous records to relevant professional standards/guidelines. Ensure patient notes are fit for purpose and information entered on the Patient Electronic Record is/stored in accordance with appropriate protocol.
  • Support the provision of EPIC Services and promote the fulfilment of the rota.
  • To monitor stock levels and ensure stock are ordered and maintained/controlled as required.

Communications & leadership

  • To attend departmental meetings as required.
  • Ensure that effective communication with patients and staff is maintained.
  • Provide reassurance to patients when providing care gaining consent by using empathy and persuasive skills.
  • Ability to communicate with patients with learning/ language disabilities.
  • Ability to communicate with challenging or confused patients.
  • To highlight any areas of concern or improvement to line management.

  • You must take reasonable care of your own health, safety and wellbeing, and that of your colleagues, in accordance with the Health and Safety policy.
  • You are required to uphold the confidentiality of all records held by SPCT, whether they are patient records, employee records or management information.
  • Comply with all SPCT policies and procedures, identifying improvements where appropriate.

  • Promote the application of standards, protocols and codes of practice as set by the Royal College of Nursing, all other Royal Colleges and Councils applicable to staff within Salford Primary Care Together, Department of Health, and all other regulatory and standard setting organisations.
  • To understand how to use the Ulysses reporting system and understand the Incident Reporting Policy and procedures, including all Safeguarding policies, procedures and reporting pathways.

Safeguarding Responsibilities

  • Safeguarding is everyones responsibility. Employees must at all times have regards for the need to safeguard, inclusive of Modern day Slavery and Human Trafficking and promote the welfare of children and adults at risk in line with legislation. Employees of SPCT must follow all safeguarding policies and procedures and reporting mechanisms to raise safeguarding concerns and keepup to date with safeguarding training as required.
